INSPECTION REPORT NOS. 50-272/95-12 AND 50-311/95-12 (REPLY)

Dear Mr. Eliason:

This refers to your August 18, 1995, correspondence in response to our letter, dated July 19, 1995, regarding Salem Units 1 and 2~ This correspondence dealt with multiple failures of electrical circuit breakers due to hardened and/or dirty grease in the operating mechanisms. A violation of the corrective action requirements of 10 CFR 50 Appendix B was cited due to your failure to identify the root cause of the failures and take corrective actions to prevent repetitive failures.

We have reviewed this matter in accordance with NRC Inspection Manual Procedure 92903, "Engineering."

You found that the root cause of the problem to be an acceptance of longstanding equipment problems as evidenced by ineffective corrective actions and the failure to perform a timely root cause analysis for the repetitive breaker failures.

Your corrective actions, including the performance of an *operability determination, maintenance procedure improvements and the performance of.

breaker overhauls and timing using the revised procedures, appear to be accepfab1e to resolve the circuit breaker failure concerns.

We also acknowledge that you are taking actions to improve the station corrective action program to avoid similar further violations.

We consider the actions you have taken to be acceptable.

We will review the.

effectiveness of these actions in a future inspection.
