Inspection Report No. 50-311/87-29

This letter refers to the team inspection conducted by Mr. A. Krasopoulos

and other NRC staff and consultants on September 14-18, 1987 at Salem Unit 2

located at Hancocks Bridge, New Jersey, of activities authorized by NRC License

No. DPR-75.

Mr. Krasopoulos discussed the findings of the inspection with

Mr. R. S. Miltenberger of your staff at the conclusion of the inspection.

This

letter also refers to a management meeting held at our office in King of Prussia,

Pennsylvania, on October 7, 1987.

The meeting was attended by you and members

of your staff and by myself and Mr. W. Kane, Director of the Division of Reactor

Projects and other members of the NRC staff.

Several commitments were made by

you at this management meeting as stated in Section 13 and Attachment 3 of the

enclosed report.

A follow-up inspection, conducted by Mr. Krasopoulos on

December 15-18, 1987, verified that the commitments made during the management

m~eting were being implemented adequately.

The findings of this inspection

were discussed with Mr. B. Preston of your staff.

Areas examined during this inspection and the follow up are described in the

NRC Region I Inspection Report which is enclosed with this letter. Within

these areas, the inspection consisted of selective examinations of procedures

and representative records, interviews with personnel, and observations by the

inspectors.

The inspections which were performed to verify compliance with the applicable

requirements of 10 CFR 50 Appendix R determined that some of your activities

were apparently not conducted in full compliance with these requirements.

The

specific apparent violations are:

The possible loss of all three emergency

diesels from a simultaneous discharge of the carbon dioxide system in the

diesel rooms; the violations identified by you in LERs 87-09 and 87-10 involv-

ing areas where a fire could damage all safe shutdown trains; a Technical

Specification violation concerning the inadequate review of a Fire Protection

Procedure; and the possible loss of radio communications during a relay room

fire.

In addition, a Deviation from a commitment to provide fire detection in

certain plant areas was identified.

The deviation is set forth in the Notice of Deviation enclosed herewi~h as

Appendix A.

You are requested to respond to the enclosed Notice, and in

preparing your response, you should follow the instructions specified therein.

Other significant issues identified during the inspection was the apparent

lack of circuit breaker coordination with associated circuits and the lack of

an evaluation to address this concern.

The NRC plans to hold an enforcement conference in the future with your staff

in Region I to discuss the results of this inspection, the potential

violations and the deviation.

We will inform you of resulting enforcement

actions in the future.

Your corrective actions regarding the identified deficiencies should not be

delayed pending either a future NRC inspection or further action by the NRC

Region I office.

The responses directed by this letter and the accompanying Notice are not

subject to the clearance procedures of the Office of Management and Budget

as required by the Paperwork Reduction Act of 1980, PL 96-511.
