Loss of Service Water Loop (21)

CONDITIONS PRIOR TO OCCURRENCE:

Mode 3 -

Rx Power 0% - Unit Load 0 MW DESCRIPTION OF OCCURRENCE:

At 1600 hours0.0185 days <br />0.444 hours <br />0.00265 weeks <br />6.088e-4 months <br /> on April 26, 1981, Service Water Pump No. 23 was declared inoperable due to packing leaks.

Since the No. 22 Service Water Pump was already inoperable for maintenance, only one Service Water Loop was operable.

Action Statement 3.7.4.1 was entered at this time.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment Failure -

The pump's main shaft packing was found to be worn.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.7.4.1 requires that with only one Service Water Loop operable, restore at least two loops to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> or be in at least hot standby within the next six hours and in cold shutdown within the.following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

CORRECTIVE ACTION

The.packing was replaced and the pump tested satisfactorily.

The Action Statement was terminated at 2022 hours0.0234 days <br />0.562 hours <br />0.00334 weeks <br />7.69371e-4 months <br /> on April 26, 1981.
