Containment Air Lock Inoperable CONDITIONS PRIOR TO OCCURRENCE:

During a routine entry into the containment, the outer door on the Containment Air Lock, elevation 100', failed to close.

At 0100 hours0.00116 days <br />0.0278 hours <br />1.653439e-4 weeks <br />3.805e-5 months <br /> on April 27, 1981, Action Statement 3.6.1.3.a was entered.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment Failure -

The Containment Air Lock Door linkage was worn, causing the door roller bearing to deviate from their proper alignment.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.6.1.3.b requires that each Containment Air Lock shall be operable with both,doors closed except when the Air Lock is being used for normal transit entry and exit through the Containment, then at least one Air Lock Door shall be closed.

CORRECTIVE ACTION

Roller bearings on the outer door were replaced in kind, and the linkage was realigned.

The Containment Air Lock was tested satis-factorily.

Action Statement 3.6.1.3.a was terminated at 0710 hours0.00822 days <br />0.197 hours <br />0.00117 weeks <br />2.70155e-4 months <br /> on April 27, 1981.
