Service Water Leak in Containment CONDITIONS PRIOR TO OCCURRENCE:

Mode 5 - Rx Power 0% - Unit Load 0 MW DESCRIPTION OF OCCURRENCE:

While performing routine shift rounds at 0500 hours0.00579 days <br />0.139 hours <br />8.267196e-4 weeks <br />1.9025e-4 months <br /> on April 7, 1981, an Equipment Operator discovered a Service Water leak emanating from the No. *24 Containment Fan Cooling Unit. In accordance with NRC IE Bulletin 80-24, the NRC was notified by telephone at 0630 hours0.00729 days <br />0.175 hours <br />0.00104 weeks <br />2.39715e-4 months <br /> with written confirmation transm~tted later in the day.

  • Investigation by maintenance personnel revealed that a vent valve (24SW63) on the Fan C~oling Unit was not fully closed. The valve was closed and the leak stopped at 1400 hours0.0162 days <br />0.389 hours <br />0.00231 weeks <br />5.327e-4 months <br />.

This occurrence constituted a degradation of a containment boundary in accordance with Technical Specification 6.9.1.8.c.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Personnel Error ANALYSIS OF OCCURRENCE:

No explanation for the valve not being fully closed can be found.

CORRECTIVE ACTION:

The leak was stopped by closing the valve.
