Dear Mr. Grier:

NRC INSPECTION REPORT NO. 50-311/78-46 NO. 2 UNIT SALEM NUCLEAR GENERATING STATION We have reviewed the report of your inspection conducted on November 20-*~2,1978, which was transmitted with your letter of December 15, 1978, and received on December 19, 1978.

Our response to the item of noncompliance is as follows:

"10 CFR 50, Appendix B, Criterion V, states, in part, 'Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.'

"The Salem Unit 2 FSAR Section D.l.6.5 requires, 'Activities that affect quality are prescribed by PSE&G and its contractors by means of written procedures, instructions, drawings or checklists

  • .. ' PSE&G procedure DTP-30-SIT-l, Part 3-13 requires that instrumentation be 'checked-out and is ready for operation' as a prerequisite to the containment structural integrity test.

"PSE&G specification 69-7437 requires that 'One month prior to proof test of building read and record all (strain) gages once each day with multichannel data acquisition system.

Measure and record realm of insulation resistance... once each week.'

"Contrary to the above, on November 22, 1978, the daily/weekly checkout of strain gages had not yet commenced, although the scheduled date for the containment proof test was November 27, 1978, with probable delay to December 9, 1978."

l/l6/79 The following affirmative actions have been initiated by PSE&G:

Corrective Action The Specification 69-7437 (Rev. 0) was written and approved for use with the Salem Unit No. l Structural Integrity Test.

. Because Unit No. 2 is a non-prototype containment, reading embedded strain gages during the SIT is not obligatory under the rules set forth in NRC Regulatory Guide 1.18.

PSE&G has elected to read a small portion of the embedded strain gages during the Unit No. 2 SIT for conforming data and as part of a separate research project.

The strain gages were read and resistance measurements recorded for a period of three weeks prior to the SIT, at approximately two-day intervals.

As such, the strain gage measurements taken were not considered a formal part of the Structural Integrity Test and, therefore, the requirements of Specification 69-7437 have been relaxed to an extent reflecting the confidence gained in these instruments during the Unit No. 1 testing program.

This relaxation has been documented in a reissue of Specification 69-7437, Rev. 1, December 8, 1978.

Future Corrective Action Since this test is non-repetitive and the problem is related only to the SIT, it is not considered symptomatic of any generic deficiency, and, therefore, no additional corrective steps are considered necessary.

Date of Compliance Full compliance was therefore attained at the date of the revised specification, December 8, 1978.
