6 H 1 Reported Under Technical Specification Date of Occurrence
>0/3~/7g Time of Occurrence pim Unit Identification and Descri tion of Occurrence:
See attached sheet.
Conditions Prior to Occurrence:
Refueling outage in progress.
Action specified in the Technical Specification Surveillance Requirements met due to ino erable e ui ment.
Describe.
None.
Refueling in progress.
A arent Cause of Occurrence:
Maintenance personnel mistakenly infected grease into the hydraulic snubbers during routine lubrication.
Anal sis of Occurrence:
Functional testing of five snubbers with grease in the oil reservoir revealed no noticeable effect on the proper operation of the snubber.
Prolonged operation with grease in the reservoir could result in an inoperable snubber.
Corrective Action; The 6 snubbers were replaced.
The alemite fittings will be replaced during the refueling outages beginning with unit l,.cycle 2.
The hydraulic snubbers in the that entry in the drywell is required.
Failure Data:
None Tennessee Valley Authority - Browns Ferry Nuclear Plant
Identification and Descri tion of Occurrence:
As a result of finding evidence of grease on two hydraulic snubbers by the NRC inspector, an inspection of all the accessible hydraulic snubbers in the three units was conducted.
Four other hydraulic snubbers were found on unit 3 with evidence of grease on the alemite fitting. All the grease appeared fresh with no evidence of dust.
Investigation revealed that about one week earlier, the annual lubrication program had been initiated.
Discussion with personnel responsible for lubrication revealed no one knowingly had lubricated the snubbers although it was evident from the discussion that if they had lubricated the snubber, they would not have wiped off the lubricant.
Review of the lubri-cation schedule revealed that the snubbers were not on the schedule.
