Enclosure to HNP 94-014 1993 ANNUALREPORT - PRIMARY COOLANTIODINESPIKES During 1993, the primary coolant did not exceed 1.0 pCi/gram dose equivalent I-131 or 100/E-bar pCi/gram gross radioactivity as set forth in Technical Specification 3.4.8.

e Enclosure to HNP 94-014 1993 ANNUALREPORT - PRIMARYREUEF AND SAFEIY VALVECHALI~GES On October 12, 1993, pressurizer power operated relief valve (PORV) PCV-444B opened and released approximately 100 gallons ofreactor coolant to the Pressurizer Relief Tank. PCV-444B

. was immediately placed in the closed position.

Excessive Reactor Coolant System pressure was not the initiating condition for this event.

The cause of the event was determined to be a failed printed circuit card in the power supply to pressure transmitter PT-444.

The failed card caused the loss of input signal to the Master Controller resulting in a high output spike before the controller failed low. The spike caused PORV PCV-444B to open.

The failed printed circuit card was replaced and was tested satisfactorily.
