The cause'was improper switching at the Pratt 6 Whitney Substati'on com-bined with incorrect wirin of both 240/138 kV autotransformer polarizing CT circuits at the Midva Substation.

The incorrect wiring has been corrected, and a ro riate personnel have been advised of the cause and NA NAS'C OF'PocPARcR N. A. Schn man corrective action for this occurrence.
