1990, of

activities authorized

by

NRC License

Nos.

NPF-41,

NPF-51 and NPF-74,

and

the discussion of .our findings held by the inspectors

with members of the

Arizona Public Service

Company staff at the conclusion of the inspection.

Areas

examined during this inspection

are described

in the enclosed

inspection

report.

Within these

areas,

the inspection consisted

of selective

examinations

of procedures

and representativ'e

records,

interviews with

personnel,

and observations

by the inspectors.

This report refers to several

instances

in which improper valve manipulations

contributed to actual

or potential

component operability problems or to the

unexpected transfer of Reactor Coolant System

(RCS) water.

The most

significant of these

include the isolation of an Atmospheric

Dump Valve

accumulator while in Mode 1, the starting of a charging

pump with the suction

and discharge

valves closed,

overpressurizing

part of the Post-Accident

Sampling System,

an

RCS spill in Unit 2,

and the inadvertent transfer of water

in the Refueling Mater Tank to the

RCS in Unit 1.

These incidents reflect a

need for more rigorous control over system manipulations.

In addition, the

loss of shutdown cooling event described

in the enclosed

inspection report and

the

RCS leak while performing repacking of a valve with the train in service

described

in Inspection

Report 50-528/90-12 point to the necessity of careful

considerations

of actions while shutdown cooling systems

are in service.

Based

on the results of this inspection, it appears

that two of your

activities were not conducted in full compliance with NRC requirements,

as set

forth in the Notice of Violation, enclosed

herewith

as Appendix A.

Two occasions

were noted in this report in which on-site

System

Engineers

identified problems with their assigned

systems.

These

instances

involved the

identification of the oil collection piping for a Reactor

Coolant

Pump not

being in the proper configuration

and the identification of paint on fuel

metering rod ports of an Emergency

Diesel Generator.

This appears

to be evidence of ownership

and

a pro-active role on the part of

your engineering staff.

We find these

observations

by your system engineers

to be significant contributions to ensuring

equipment operability,

and

encourage

continuation of this level of performance.

Your response

to this Notice is to be submitted in accordance

with the

provisions of 10 CFR Part 2.201

as stated

in Appendix A, Notice of Violation.

In accordance

with 10 CFR Part 2.790(a),

a copy of this letter and the

enclosures will be placed in the

NPC Public Document

Room.

The response

directed

by this letter and the accompanying

Notice are not

subject to the clearance

procedure of the Office of Management

and Budget

as required

by the Paperwork Reduction Act of 1980,

PL 96-511.
