APPENDIX A NOTICE OF VIOLATION Arizona Public Service Company Palo Verde Units 1 and 3
Docket Numbers 50-528 and 50-530 License Numbers NPF-41 and NPF-74 During an NRC inspection conducted on April 15 through Hay 26,
- 1990, violations of NRC requirements were identified.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions,"
10 CFR" Part 2, Appendix C (1990), the violations are listed below:
A.
Unit 1 Technical Specification 6.8. 1 states, in part: "Written procedures shall be established, implemented, and maintained covering... Surveillance and test activities of safety-related equipment,"
B.
Surveillance procedures are implemented in part by surveillance test Procedure 74ST-9SS02, "Post Accident Sampling System Leakage Monitoring."
This procedure requires in step 7.5 verification that "the RDT is available to receive PASS effluents."
This is accomplished by performing steps in Operating Procedure 740P-1SS02, "Operation of the Post Accident Sampling System", in which Step 4.3. 1.2 states, "for RDT return, request Control Room open/check open 1-CHUV-715. "
Contrary to the ab'ove, on April 22, 1990, while performing step 7.5 of 74ST-9SS02, valve 1-CHUV-715 in Unit 1 was not opened and subsequently resulted in over-pressurizing part of the Post Accident Sampling System.
This is a Severity Level IV violation applicable to Unit 1 (Supplement I).
Unit 3 Technical Specification 6.8.1 states, in part: "Written procedures shall be established, implemented, and maintained covering... the recommendations in Appendix A of Regulatory Guide 1.33, Revision 2, February, 1978..."

RG 1.33, Section 1.c.,
"Equipment Control (e.g.,locking and tagging)," is implemented, in part, by procedure 40AC-OOP06, "Locked Valve and Breaker Control," Revision 7.
Appendix A of this procedure requires that valve SGE-V-363 be in the locked open position.
Contrary to the above, on April 22, 1990, after implementation of procedure 40AC-OOP06, valve SGE-V-363 for Atmospheric Dump Valve 185 in Unit, 3 was found in the locked closed position.
This is a Severity Level IV violation applicable to Unit 3 (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, Arizona Public Service Company is hereby required to submit a written statement or explanation to the U.
S.
Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, DC 20555, with a copy to the Regional Administrator, Region V, and a copy to the NRC Resident Inspection office at the Palo Verde Nuclear Generating Station, within 30 days of the date of the letter transmitting this Notice of Violation (Notice).
This reply should be clearly marked as a "Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation, or if contested, the basis for disputing the violation (2) the corrective steps that have been taken and the results achieved; (3) the corrective steps that will be taken to avoid further violations; and (4) the date when full
'compliance will be achieved.
If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked or why such other action as may be proper should not be taken.
Where good cause is shown, consideration wi11 be given to extending your response time.
