V Office in Walnut Creek,

February 29, 1988.

The subjects

discussed

are

summarized

in

Nos. 50-528/88-11,

50-529/88-11,

and 50-530/88-11,

enclosed

The engineering errors which led to the steam driven auxiliary feedwater

pumps in Units

1 and

2 being inadvertently rendered

inoperable,

and

the results of our recent Safety System Functional

Inspection,

again

reenforce

our conclusion that you need to focus attention

on ensuring

the establishment

of a strong, active engineering organization.

We find

this particularly vexing in view of the assurances

we have received

from ANPP management

over the past several

years that the quality

of technical

work was being given prioritv attention.

The focus of the enforcement

conference

was the circumstances

surrounding

the

steam driven auxiliary feedwater

pumps at Units

1 and

2 being rendered

inoperable,

as described

in your Licensee

Event Report 50-528/87-025.

The

management

meeting discussion

centered

on several

recent events at Palo

Verde, including an inadvertent safety injection at Unit 2, the inoperable

Post Accident Sampling System event at Unit 2, the introduction of foreign

material into the Unit

1 reactor internals

which led to

a stuck control rod,

and the mispositioning of the discharge

valve for the non-safety auxiliary

feedwater

pump at Unit 2.

Our concerns,

as expressed

to you during this

meeting,

can

be summarized

as follows:

Recent events

and observations

at Palo Verde are indicative of a

divergence

between

management

expectations

and personnel

performance.

Several of the events

discussed

have in comon multiple personnel

errors.

Additionally, our review indicates that your senior managers

are only

infrequently touring important areas

of your facility.

Our experience

is

that frequent, critical observations

of plant activities

and direct

discussions

hetween

personnel

at all levels is required for management

to

realistically have

knowledge of how day-to-day business

is conducted

and to ensure

personnel

understand

what is expected.

We encourage

you to

reassess

the degree to which your managers

dedicate

personal

time to

observing plant activities.

we are encouraged

that you have formulated long-term plans to enhance

your engineering capability,

we believe that increased

direct management

attention

towards work presently

being performed

by your engineering

staff is advisable.

Your Quality Assurance

organization

does

not appear

to be at the

forefront in identifying problem areas

and then ensuring that

a proper

resolution is reached.

As discussed

in our meeting,

we encourage

you to

ensure that Quality Assurance

plays

a strong role in the operation of

Palo Verde and hold Quality Assurance

accountable for failure to identify

problem areas.

With regard to the actions

you described

to improve the utilization of your

simulator and to more closely follow your backlog of maintenance

items,

we

find your efforts to be positive.

As described

in the attached

meeting report,

we anticipate discussing

these

concerns,

and other issues

documented

in this 'report, at

a future

NRC/ANPP

meeting.

As with our 'past meetings,

we found this meeting to be beneficial

and look forward to continuing this dialogue.

In accordance

with 10 CFR 2.790(a),

a copy of this letter and the enclosures

will be placed in the

NRC Public Document

Room.
