This letter refers to the Inspection conducted by Messrs.

P. Norbut of your staff on October 22-26,

1979, as documented in your letter of January 16, 1980 of activities authorized by Nuclear Regulatory Commission (NRC) Construction Permits Nos.

CPPR-141, 142 and 143.

During this Inspection, one item of noncompliance was identified.

Our response to this item of noncompliance (Infraction) is presented in the enclosed Attachment A.

CPPR-141, CPPR-142 Notice of Violation Based on the results of the NRC Inspection, conducted on October 22-26, 1979, it appears that one of your activities was not conducted in full compliance with NRC requirements as indicated below:

10CFR50, Appendix B, Criterion V, Instructions, Procedures and Drawings, as implemented by PSAR Section 17.1A.5, re-quires that activities affecting quality be accomplished in accordance with instructions, procedures or drawings appropriate to the circumstances.

Contrary to the above requirement, on October 24, 1979, the NRC Inspector identified undersized fillet welds for the column to base welds for the installed support assemblies of the Unit 81 and 82 low-pressure safety injection pumps and containment spray pumps.

Specif-ically, seventeen (17) of the forty-eight (48) fillet welds for the four (4) Unit IIl pump support assembly columns and sixteen (16) of the forty-eight (48) fillet welds for the four (4) Unit II2 pump support assembly columns to base plate welds were undersiz,ed com-pared to the size specified in Construction Drawing 13-C-ZAS-242.

This is an Infraction.

Corrective Ste s Which Have Been Taken On October 25,

1979, NCR C-A-1592 was initiated to document results of QC inspection on Units I/1 and 82 pump support assemblies fabricated by Marathon Steel to Specification 13-CM-131.

Reinspection of all welds found six (6) Unit 81 and two (2) Unit !/2 pump stands with undersized welds not meeting AWS Dl.l requirements for 5/16" fillets.

The NCR was dispositioned to repair welds using Bechtel Weld Proce-dure Pl-A-Lh (Struct)R3 and inspect per 13-CM-320.

Accep-tance of repairs was completed on January 9,

1980 for Unit f11 and February 4, 1980 for Unit /I2.

Combined QA/QC surveillance was performed on February 6,

1980 on Unit 83 LPSI pump support assemblies for undersized welds.

All welds were found acceptable to drawing require-ments for fillet weld size/length, using appropriate fillet gauges.

Corrective Ste s Which Have Been Taken to Avoid Further Violations The Supplier's Quality Control personnel and Bechtel's Supplier Quality Representative (SQR) were remiss in not detecting the undersized fillet welds prior to shipment.

Project Trend Analysis Program further identified similar weld defects in items furnished by Marathon, Steel to 13-CM-131.

On November 29,

1979, QA issued CAR No. S-79-145 to obtain correc-tive action from the Supplier and Bechtel Procurement Supplier Quality Department (PSQD) to prevent recurrence.

On January 18, 1980, this CAR was closed out based on satisfactory response that included:

Letter from PSQD confirming written instruction to SQR and Area Supervisor reiterating necessity to increase surveil-lance activities on in-process/final inspection for fabri-cation to design drawings and conformance of welds to AWS D1.1 and Specification 13-CM-131.

Letter from Marathon Steel, dated December 19,

1979, summarizing actions implemented to preclude recurrence of

undersized welds on materials furnished to PVNGS.

This letter documents meetings with Shop/QC Supervision to emphasize proper inspection methods and measurement cri-teria of welds, issuance of fillet weld gauges and instruc-tions for their use and increased random checks of their 100% visual inspection program.

Subsequent Trend Analysis for the past two report periods indicate no further incidents or repetition of welds fail-ing to meet requirements.

APS/Bechtel meetings were held November 29, 1979 (Downey) and December 20, 1979 (Deer Valley) with appropriate Quality Assur-ance and Supplier Quality personnel to follow-up on certain

actions, which included Marathon Steel performance.

Letter ANPP-14655, dated January 17,

1980, was initiated to Bechtel Project Management to expedite resolution to identified areas of concern.
