DRESDEN UNIT 3 -
SAFET~ SYSTEMS OUTAGE MODIFICATIONS INSPECTION This letter discusses the results to date of a safety systems outage modificatior installation and testing inspection, which is being conducted at the Dresden Unit 3 site by *a team from the NRC's Office of Inspection an... d Enforcement.
This installation and testing inspection is part of the Safety Systems Outage Modifications Inspection (SSOMI) program.
Results of the earlier design and procurement inspectio~s are being reported separately.
The first part of the installation arid testing inspection was conducted during *the period April 21 to May 7, 1986; the final part of the inspection is presently scheduled for June 9 to 25. 1986, contingent upon the Unit 3 outdge schedule.
- During the inspection to date, the inspection team has identified several c0ncerns that warrant CECo management attention to ensure that, prior to restart of the unit, effective corrective actions are taken relative to.the specific hardware deficiencies identified, and CC'mprehensive corrective actions are initiated relative to the orogrammatic defidencies identified.
The areas of concern identified by tht inspection team are discu~sed below.
Detailed infonnation regarding each deficiency identified was presented to CECo station per~onnel during the inspection, and each area of concern was discussed with C£Co station management during the inspection and at the briefing on May 7, 1986.

Improper Installation of Eguip~ent.
Significant installation problems were identified in nineteen of the twenty four modifications inspected by the team.
Examples of identified installatio~ problems were the reversed installation of ~h~ a~tuator lines.
for the torus to reactor building vacuum breaker valves, incorrect installation of the LPCI room cooler.motor, improper Raychem cable splices associated with eight safety-related components. tennination of the wrong cables for the core spray suction valve ~edification, and bundling of safety-related caoles with non safety-related cables in the analog trip system cabinets.

building wall, changes to the materials, configuration and stroke times of safety-related valves, and changes to the motor connections for the LPCI room.* cooler modification. Discussions by the team with maintenance
'personnel resulted in.a perce~tion that those personnel consider the procedures and draw1ngs in the modifications* packages to be guideline~
rather than.requirements. This perception was reinforced by the number of instances identified 1n which significant deviations from the installation instructions were made in the field with no feedback to allow engineering -
-review of the changes.
Inadequate Procedures and Instructions.
- The.team identified significant procedural deficiencies in twenty-two of the twenty-four modifications inspected. Examples of deficiencies include~ lack of acceptance criteria for electrical tests~* conflicting Technical Specification requirements for the LPCI room cooler* installa-tion, lack of procedures for installation of lead shielding, an unworkable*
test procedure for the vacuum breaker valve solenoid modification and;*.
_*contrary to a 1983 Confirmatory Action Letter from Region III, five work requests with instructions for greasing valve seats.
Inadequate QA/QC Overview of Modification Activities.
The numbers and types of installation and procedural deficiencies identi-fied by the.team during the inspection indicate that the QC coverage of installation activities, the QA overview of the QC inspections, and the*
CECo review of procedures and records, need to be significantly impro~ed.
The major installation and docuraentation deficiencies not~d above, and a number of other significant deficiencies identified by the team, had been prtviously signed off by QC as acceptable.
The inspection team and Region III will review your corrective actions on specific deficiencies during the final phase of the installation and testing inspection in June and, during subsequent inspections, will review your correc-tive actions relative to the identified programmatic deficiencies.
