Dear Sir or Madam:

During Refueling Outage R22, an indication was found by ultrasonic testing (UT) examination in A Residual Heat Removal Low Pressure Coolant Injection Loop weld number 24-10-130.

Pursuant to 10 CFR 50.55a(z)(1), James A. FitzPatrick Nuclear Power Plant (JAF) proposed a repair in accordance with Relief Request RR-21 [Reference 1] and included a commitment to submit information to the NRC regarding the results of the Weld Overlay repair. This letter submits results of UT examination of the Weld Overlay.

(1) Weld Overlay examination results including a listing of indications detected, if any; (2) Disposition of indications using the standards of ASME Section XI, Subsection IWB-3514-2 and/ or IWB-3514-3 criteria and, if possible, the type and nature of the indications is the final examination report for the Weld Overlay and a listing of the indications detected. The report contains disposition of indications in the Weld Overlay using the acceptance standards of ASME Section XI, Subsection IWB-3514-2 and/or IWB-3514-3 criteria and the type and nature of the indications. A total of eight indications have been dispositioned using ASME Section XI, Subsection IWB-3514-2 and IWB-3514-3 and ASME Section XI Appendix Q (Q-4100(c)). Five laminar flaws were evaluated per ASME Section XI, Subsection IWB-3514-3 and three planar flaws were evaluated per ASME Section XI, Subsection IWB-3514-2.

JAFP-17-0023 Page 2 of 2 (3) Discussion of any repairs to the weld overlay material and/or base metal and the reason for the repairs; and Prior to the installation of the weld overlay, base material indications were identified in the cast carbon steel valve body during the liquid penetrant testing (PT) examination. Indications in the cast carbon steel valve body beneath the Weld Overlay or within W' of the toe of the Weld Overlay were removed or reduced to an acceptable size to comply with ASME Section XI Code case N-504-4, paragraph (c) and Appendix Q, paragraph Q-2000 and Q-4100.

During installation of the Weld Overlay buffer layers, the detected axial indication in the dissimilar metal weld created a through wall leak. A manual shielded metal arc welding (SMAW) repair patch was applied over the through wall leak and repair of unacceptable indications in buffer layers was performed.

On 2/11/2017, eighteen (18) indications were identified in the Weld Overlay UT examination (Enclosure 2). The weld overlay repair consisted of mechanically removing a 360 degree area on the carbon steel valve side of the weld overlay and an area at top dead center of the stainless steel Tee side of the weld overlay.

The 360 degree repair cavity required reapplication of the temper bead layers due to the depth of the repair cavity. The stainless steel Tee repair required manual reapplication of the ER308L buffer layers. The remaining Alloy 52M was applied via machine Gas Tungsten-Arc Welding (GTAW).

Pages 3 and 4 of Enclosure 1 disposition the remaining indications in the final Weld Overlay after the Weld Overlay repair.

(4) Description of the examination coverage achieved during the examination of the Weld Overlay. documents the examination coverage achieved during the examination of the final Weld Overlay. Page 7 of Enclosure 1 states the Total Weld Overlay Material coverage was 96.0% and the Total ISi Volume Coverage was 96.1 %.

This report documents the results of the Weld Overlay Material examination for the exam volume depicted in Figure Q-4100-1 of ASME Section XI Appendix Q.

Examination performed to the requirements of ASME BP&V Code,Section XI, 2001 Edition with 2003 Addenda and the requirements of Relief Request RR-21 .

Multiple Recordable Indications were noted within the examination volume of the WOL material. Indications included both laminar and planar flaws.

Laminar flaws #1 , 2, 3, 4, 6, 7, 8, 9, 10 and 11 exceed the acceplance criteria ofTable IWB-3514-3 and the conditions imposed on Code Case N-504-4 per RG 1.147 R17.

Laminar flaw #5 Is Rejectable based on the maximum postulated planar flaw residing under the obstructed volume exceeding the acceptance criteria of Table IWB-3514-2.
