This letter transmits
the results of the special
safety inspection
conducted
by Mr. W. Cook and Mr.
R.
Laura of this office on December
3,
1988 thru
December
21,
1988 at Nine Mile Point Unit
2, Scriba,
A summary of
the findings was presented
to Mr. J. Willis and other members of your staff at
the conclusion of the inspection.
This inspection
was conducted
to review the circumstances
which 'led to the
discovery,
by your staff, that the
Division
. 1 Automatic Depressurization
System
(ADS) logic circuitry for Unit 2 was found to be inoperable
due to
a wiring
error since initial fuel load on November 2,
1986.
ADS is an
Emergency
Core Cooling System
(ECCS) designed
to mitigate the
consequences
of an accident
by automatically reducing reactor pressure, if
necessary,
to the discharge
head of the
Low Pressure
Core Injection and Spray
Systems.
Operation of the unit in Modes 1,2 and
3 (Power Operations,
Startup
and Hot Shutdown) with Division I ADS inoperable
is an apparent violation of
Technical Specification 3.3.3.
We intend to schedule
an enforcement
conference
to discuss this matter
and
will contact you to arrange
a mutually agreeable
time and date.
At the
conference
you should
be prepared
to discuss
the causes
of and corrective
action for this event,
as well as the safety significance of the
configuration.
You should also
be prepared
to discuss
why the operational,
star tup and periodic surviellance testing failed to properly detect this
deficiency,
and action you have taken to ensure that there
are
no other
similar wiring problems.
