Cook and Mr.
W. Schmidt
and others of this office on April 1,
1988 to
May 5,
1988 at Nine Mile Point Units
1 and 2, Scriba,
A summary of
the findings was presented
to Mr. J. Willis and other members of your staff at
the conclusion of the inspection.
The enclosed
Notice of Violation (Appendix A) is being issued to address
three
problems:
1) failure to prevent the recurrence
of missed firewatch patrols;
2) failure to conduct
sampling prior to purging the drywell; and 3) failure to
perform leak rate testing
on
a containment penetration.
These violations have
been categorized
by severity level according to
10 CFR Part 2, Appendix
C
You are required to respond to these violations and in
preparing your response
you should follow the instructions
in Appendix A.
In
your response
you should include those actions
taken or planned to improve
management
oversight of these
areas.
As described
in the attached
report
(Section
1. l.b and 1.2.a) previous corrective actions
were ineffective.
The response
required
by this letter is not subject to the clearance
procedures
of the Office of Management
and Budget
as required
by the Paperwork
Reduction Act of 1980,
PL96-511.
