PILGRIM NUCLEAR POWER STATION - NOTIFICATION OF INSPECTION PROCEDURE 95003 PHASE C INSPECTION
Dear Mr. Dent:
The purpose of this letter is to notify you that U.S. Nuclear Regulatory Commission (NRC) staff have received your formal declaration of readiness for Phase C of Inspection Procedure (IP) 95003, dated September 2, 2016 (ML16258A004)1. We have scheduled the on-site portions of this inspection for November 28 - December 9, 2016, and January 9 - 13, 2017. This inspection will consist of 20 inspectors from across all the NRC regional offices, as well as the agencys headquarters office. The information provided in this letter supplements, but does not supersede, the inspection plan information included in the mid-cycle assessment letter, dated August 31, 2016 (ML16239A119). An updated inspection plan is attached.
The objectives of IP 95003 are to: 1) provide the NRC with timely additional information to be used in deciding whether the continued operation of the facility is acceptable and whether additional regulatory actions are necessary to arrest declining licensee/plant performance; 2) provide an independent assessment of the extent of risk-significant issues to aid in the determination of whether an unacceptable margin of safety or security exists; 3) independently assess the adequacy of the programs and processes used by the licensee to identify, evaluate, and correct performance issues; 4) independently evaluate the adequacy of programs and processes in the affected strategic performance areas; 5) provide insight into the overall root and contributing causes of identified performance deficiencies; and 6) evaluate the licensees third-party safety culture assessment and conduct a graded assessment of the licensees safety culture based on the results of the evaluation.
In a letter dated December 18, 2015 (ML15352A128), the NRC informed you of its intent to conduct the IP 95003 inspection in three phases (A through C). The Phase A and B portions of the inspection are complete (ML16060A018 and ML16144A027). operation was acceptable or additional regulatory action was needed to arrest declining performance. Based on the results of these inspections, the NRC concluded that Pilgrim continued to operate safely.
Phase C of the inspection will be focused on the Pilgrim corrective action program and safety culture assessment (reference sections 02.02 and 02.07 through 02.09 of IP 95003). The scope will also include the Reactor Safety Strategic Performance Area, with focus on the key attributes of human performance, procedure quality, and equipment performance (reference sections 02.03c, 02.03d, and 02.03e of IP 95003). Additionally, the NRC will review the root cause, extent-of-cause, and extent-of-condition evaluations associated with the White finding related to performance of the A safety relief valve (ML15230A217).
In accordance with Title 10 of the Code of Federal Regulations (10 CFR) 2.390 of the NRCs Rules of Practice, a copy of this letter will be made available electronically for public inspection in the NRC Public Document Room or from the Publicly Available Records component of the NRC's ADAMS.
