1.0 INTRODUCTION
1.1 Purpose The purpose of this procedure is to provide for the timely notification of appropriate individuals or organizations when the Shift Manager or the Site Emergency Director (SED) has determined through the use of EPIP-1 that an event has occurred which is classified as a General Emergency. Additionally, this procedure provides for periodic evaluation of the current situation by the Shift Manager/SED to determine whether the General Emergency should be continued or terminated.

CAUTION Notification of the NRC shall be completed immediately after notification of the appropriate State or Iocal agencies and not later than one hour after the time of Emergency Classification.

3.8 Monitor / Re-evaluate the Event t1] Monitoring and reevaluation of plant events along with communicating significant changes should be performed continuously as a function of the emergency response. Methods used to communicate significant changes are not formalized and may vary depending upon staffing levels as wel! as availability of personnel or equipment.
Aooendix E provides a systematic approach to monitoring/re-evaluation and the communication of significant changes in plant conditions.

121 Reevaluation of significant changes must additionally include a determination of whether Protective Action Recommendations (PARs) should be upgraded.
The need to upgrade PARs is determined through the continuous assessment of Aooendix H "Protective Action Recommendations".

4.2 Non.QA Records t1] Emergency Records The records generated due to declaration of an emergency classification are considered Lifetime Retention records. These records shall be fonrarded to the WBN EP Manager. The records necessary to demonstrate performance are then submitted to the Corporate EP Manager for storage.
l2l Drill and Exercise Records The records deemed necessary to demonstrate performance of key actions during drills are considered Non-QA records. These records shall be forwarded to the WBN EP Manager. The WBN EP Manager shall retain records necessary to demonstrate eight-year plan requirements for eight years. The WBN EP Manager shall retain other records in this category for three years.

- 1. The TSC is an area within the plant near the control room dedicated for use during an emergency. The TSC will be the focal point of onsite activity during the event. The TSC will have sufficient staff to provide management control of the site response to the event. Equipment will be available to enable the TSC staff to communicate with onsite and offsite TVA emergency personnel.
- 2. An area within the TSC will be dedicated for NRC use and will include five telephone sets and the NRC FTS 2000 System telephones.
- 3. The TSC will have the same habitability as the control room.
- 4. Sufficient plant parameter information will be available to the TSC to enable the TSC staff to assess the consequences of an event and assist the control room personne! in mitigating the accident.
- 5. Sufficient information will be transmitted to the CECC to enable the CECC Director to make protective action recommendations to State authorities.
- 6. Activation time for the TSC is approximately 90 minutes following declaration of an Alert or higher emergency classification, depending upon the time of day, weather conditions or immediate availability of personnel.
- 7. The Site Emergency Director (SED) shall assume responsibility for implementing this procedure and directing TSC personnel and activities.
- 8. The SED is responsible for the timely activation of the TSC.
NOTE ln the event of plant inaccessibility, all references to the TSC (or OSC) are intended to refer to the alternate location selected for staffing, such as the Alternate Facility in the PSO Building.
B. The TSC will provide the following functions:
- 1. Provide plant management and technical support to plant Operations personnel during emergency conditions.
- 2. Perform CECC functions for the Alert Emergency class, the Site Area Emergency class, and General Emergency class untilthe CECC is functional.

- 3. Help the reactor operators determine the plant safety status.
- 4. Relieve the reactor operators of peripheral duties and communications not directly related to reactor system manipulations.
- 5. Prevent congestion in the control room.
- 6. Provide assistance to the operators by technical personnelwho have comprehensive plant data at their disposal.
- 7. Provide a coordinated emergency response by both technical and management personnel.
- 8. Provide reliable communications between onsite and offsite emergency response personnel.
- 9. Provide a focal point for development of recommendations for offsite actions.
- 10. Provide relevant plant data to the NRC for its analysis of abnormal plant operating conditions.
C. lf the TSC becomes uninhabitable, the SED will relocate the TSC to an alternate location based on RP/OPERATIONS advice.
D. Log Keeping
- 1. lndividual logs and facility logs are legal records of activities that occurred during an emergency. lt is vital that they are as complete as possible. Web EOC electronic logs can be accessed on the EP web site and should be used when applicable.
- 2. Logs should include such information as:
- a. Times of major events and subsequent actions taken (such as, change in emergency classifications, fission product barrier status, discovery of an unplanned radiological release).
- b. Times and content of important communications with other members of the ERO that are related to major events (such as, decisions made during turnover or routine briefings and subsequent actions taken).
- c. Specific actions taken to mitigate equipment failures, contain chemical or radiological spills or fires, and so forth.

- d. Specific references to Emergency Radiation Work Permits, clearances, procedure deviations authorized emergency radiation exposures authorized, and so forth.

- a. lnitiate the last sheet of the applicable checklists as follows:

- b. Enter chronologically those events that are pertinent to the particular individual or organizations:
(1) TIME: Record the time (using the 24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> clock) that a message or information was received or action was taken.
(2) COMMENTS: Briefly record the incident, message, or order received or transmitted. lndicate the time of the incident and actions taken.
- c. Upon relief from the position or termination of the emergency, complete the log as follows:
(1) PAGE OF: Enter the total number of pages used at the top of each page (that is, Page I of 12, Page 2 of 12, and so forth).
(2) NAME AND SIGNATURE: Check the log for completeness, then in the Comments Section of the last page used in the log, print and sign your name.
- d. The person relieving the position will initiate and maintain a new log and review any previously prepared logs to allow for continuity of the position.
- e. Upon termination of the emergency or exercise/drill, provide all completed logs to the EP Manager.

- 1. Upon declaration of an Alert, Site Area Emergency or General Emergency classification.
- 2. At the discretion of the Shift Manager (SM)/Site Emergency Director (SED).
B. The Shift Manager will initiate activation of the TSC by announcing the emergency condition using one or more of the following methods.
- 1. Plant Public Address (PA) announcement.
- 2. TEENS or Emergency Paging System (EPS) activation.
NOTE The Radiological Emergency Response Organization Call List is managed in accordance with the TVA Fitness For Duty Program.
- 3. Utilizing the Radiological Emergency Response Organization Call List and Appendix V of this procedure.
3.3 Activation of the TSG A. Target activation time for the TSC is approximately 90 minutes. Once notified, Emergency Responders are expected to report to the TSC without delay.
B. The SED will declare the TSC activated once responders for the required minimum staffing positions are identified as present in the TSC and shall inform the Shift Manager of the final transfer of responsibilities.
C. Once the TSC is activated a plant-wide brief will be given announcing the facility activation, transfer of SED responsibilities from the SM to the TSC and updating personnel on the status of the emergency.
D. Positions that are in communication with the CECC will contact their CECC counterparts as soon as practical. TSC personnel should not let contacting the CECC delay activation of the TSC.
E. Upon arrival in the TSC, personnel will perform the following
- 1. Sign in on the roster and the TSC Staffing chart.

Refer to their appropriate position notebook and appendix to this procedure and perform the activities listed in the appropriate lnitial TSC Activation Checklist (appendices D-R). lf relieving a position in the TSC that is previously activated and staffed, completion of the activation checklist is not required. Receive turnover and log turnover in accordance with this procedure.
- 3. Turn on and perform function checks of equipment they will be using.
- 4. Report readiness status to the EP Manager when activation checklist is complete.
NOTE All TSC responders shall have unescorted protected area access and shall comply with fitness-for-duty policies while on-call.

- 1. The SED will periodically review the plant conditions and update the priorities for mitigating the plant emergency.
- 2. These priorities will be communicated to the ERO during periodic briefings and on the SED Priorities Status Board.
- 3. The SED may assign tasks to functional areas in the TSC. These tasks will be tracked by the TSC Logkeeper and the TSC organization the task is assigned to using logs and status boards as appropriate.
B. Briefings
- 1. lnitial Facility Briefing An initial briefing should be provided to the facility staff as soon as possible.
- 2. Periodic Facility Briefings
- a. lntra-facility briefings should be held at least once per hour using the Plant PA or the TSC/OSC PA system.
- b. The SED, if available, will initiate the briefings.
- c. lf the SED is not available, then the EP manager or the Operations Manager will initiate the briefings.
- d. Briefings will use Appendix B, Facility Briefing Guide, as a guide.
- 3. lmpromptu Facility Update Anyone in the TSC that comes into possession of important information should conduct a facility update by first gaining everyone's attention and then making an announcement.
C. Requesting Tasks/Actions from other Facilities (OSC, Control Room or CECC)
- 1. Appendix C, Action Request Form, will be used to request tasks be performed by another facility.

- 2. The person initiating the request will check, as appropriate, this is a driil or this is an actual emergency.
lf the request is from the Main Control Room, then the Operations Communicator will initiate the request.

- 4. The initiator will give the form to the TAM or Operations Manager for review.
- 5. The TAM or Ops Manager will decide if the request is for a Rapid Response Team.
- 6. The form will be approved or rejected by the SED or designee, who will record this in the disposition section.
- 7. The form will be provided to the TSC Logkeeper, who will review the form for completeness and assign a tracking number to the request (Request Number).
- 8. The Logkeeper will give the form to the appropriate team member for disposition.
- 9. lf the request is an OSC task, the Maintenance Manager will call the OSC Board Writer to discuss.
- 10. The form will be faxed to the appropriate facility.
- 11. The completed forms will be collected by the Maintenance Manager.
D. Types of teams dispatched from the OSC
- 1. TeamA
- a. Team A will consist of an electrician and an Assistant Unit Operator (AUO) that will be dispatched to the Main Control Room to assist with plant shutdown issues. They will be tracked on the teams status board and operate under the control of the Main Control Room. Team A should be considered a Rapid Response Team.

- b. The primary responsibilities of Team A are to respond to instructions that require operations within the shutdown board rooms, (those electrical board rooms located on Auxiliary Building, elevations 757' and772'); and any tasks that would be in the Control Building proper.
This allows the MCR to perform necessary actions in a timely manner that is required for unit shutdown. Team "A" should not be sent anywhere else for additional tasks, i.e., Reactor Building, Turbine Building, etc. The Shift Manager or his designee would have responsibility for these individuals accountability and would notify the TSC/OSC of their locations and all actions taken by this team, as required.
- 2. Rapid Response Team A Rapid Response Team may be assembled and dispatched with a minimal briefing in order to perform time critical operator actions, evolutions or assessments of plant conditions. A Rapid Response Team may include an RP Technician to provide job coverage in lieu of a detailed RP brief.
Examples of rapid response actions may include the following:
- a. opening reactor trip breakers following an Anticipated Transient Without Scram (ATWS)
- b. medical emergency
- c. fire/appendix R fire
- d. providing system alignments for core cooling
- e. establishing water inventory for core cooling
- f. mitigating LOCAs
- g. mitigatingradiological releases
- h. restoring vital/shutdown power supplies
- i. performing radiological surveillances for dose assessment
- j. determining radiological release pathways
- k. realigning steam supply to TDAFW pump L closing MSIVs or PORVs to prevent release from tube rupture
- m. SED/TSC discretion to restore control of the plant

F. Facility Habitability
- 1. Radiation Protection will conduct periodic radiologica! habitability surveys to ensure the continued safety of TSC occupants.
- 2. Unsatisfactory habitability surveys may indicate a need to suspend eating, drinking and smoking in affected plant areas or move the TSC to an alternate location based on RP/Operations advice.
G. Severe Accident Management

B. The SED will notify the Central Emergency Control Center (CECC) of the decision to begin long{erm operation.
C. Meals and arrangements for sleeping facilities will be made at the request of the SED. These arrangements may be made by the CECC.
D. Additional personnel will be called in at the request of the SED to provide coverage or to ensure 12-hour or shorter shifts in the TSC. The SED will coordinate these call-ins with Nuclear Security to facilitate site access.
E. The SED, through the OSC Manager, will establish 12-hour (or shorter) shifts for craft personnel onsite and call in additional personnel as necessary.
3.6 Normal Work Process Deviation A. When a Site Area Emergency or General Emergency has been declared, deviations from normal processes may be authorized by the Site Emergency Director, or designee, in order to take action to restore core cooling, mitigate an off-site release in progress, or maintain spent fuel pool cooling. Evaluation of deviations from the normal established processes should be considered prior to dispatch of work.
B. Attachment 5 of NPG-SPP-18.3, SED Procedure Deviation Checklist, provides a mechanism to evaluate and document such deviations.
3.7 Termination and Deactivation A. REFER TO EPIP-16, "Termination of the Emergency and Recovery," for activities associated with terminating emergencies, TSC deactivation, and post-accident recovery.
B. All equipment, supplies, and procedures will be replenished in the TSC following a drill, exercise or emergency by applicable groups as assigned in EPIP-12,
