PORV AND SAFETY VALVE LIFT FREQUENCY AND MECHANICAL RELIABILITY By our letter dated August 21, 1979, you were provided with a listing of long term generic issues related to the Commission Orders of May 1979. Item No. 6 of Enclosure 1 to this letter stated that licensees would be directed by letter to take further action on resolving the matter of PORV and safety valve lift frequency and mechanical reliability. The purpose of this letter is to provide you with the staff requirements for resolution of this matter.
With regard to lift frequency of the PORV and safety valves, please provide responses to the enclosed request for additional information. Responses to these questions will be used to assist the staff in evaluating the probability of challenging the PORV as a result of the revised setpoints for the high RCS pressure trip and PORV actuation. The information requested in the enclosure was discussed with the B&W Owners' Group (TMI-2 Effects Subcommittee) on September 13, 1979. It is requested that these responses be provided by October 15, 1979.
With regard to the mechanical reliability of the PORV and safety valves, this concern was discussed in Section 8.4.6 of NUREG-0560 ("Staff Report on the Generic Assessment of Feedwater Transients in Pressurized Water Reactors Designed by the Babcock & Wilcox Company," May 1979).
This matter was further discussed in the staff's evaluation of each licensee's compliance with its Commission Order of May 1979. The staff requirements for resolving this issue have been superseded in scope and schedule by Section 2.1.2 of NUREG-0578 ("TMI-2 Lessons Learned Task Force Status Report and Short-Term Recommendations," July 1979). The implementation schedule for this item may be found in Enclosure 6 to our letter to all operating plants dated September 13, 1979.

REQUEST FOR ADDITIONAL INFORMATION ON PORV ACTUATION AND REACTOR TRIP FREQUENCY
- 1. According to statements made by B&W, there are approximately 146 documented occasions where PORV actuation occurred at B&W facilities prior to the accident at Three Mile Island, Unit 2 (TMI-2). For each of these events which have occurred at your facility(ies), provide the following information:
- a. The cause of.the event;
- b. The initial power level prior to the transient;
- c. Indicate which of these transients caused the reactor to trip on high RCS pressure and/or caused the safety valves to lift; and,
- d. If you assume that the present setpoints for high RCS pressure trip and PORV actuation were in effect at the time of each of these transients, estimate whether either of the following would have taken place:
(1) PORV actuation, and (2) lifting of the safety valves.
(For this item assume no credit for the anticipatory control-grade reactor trip on loss of feedwater or turbine trip.)
- 2. Provide a complete listing of reactor trips for your facility(ies) which have occurred subsequent to the revised setpoints for PORV actuation and high RCS pressure trip. This listing should include the following items:
- a. The cause of each event;
- b. The initial power level prior to the transient;
- c. Indicate which of these transients caused the PORV and/or safety valves to open; and,
- d. If the old (pre-TMI-2) setpoints for high RCS pressure and PORV actuation were in effect at the time of these transients, estimate whether any or all of the following would have taken place:
(1) PORV actuation; (2) reactor trip on high RCS pressure, and (3) lifting of the safety valves.
- 3. Provide an estimate of the increase in reactor trip frequency since lowering the high pressure trip setpoint and adding the anticipatory reactor trip.
Include a review of the design criterion for the number of reactor trips over the plant life and evaluate the effect of the increase in trip frequency on these criteria. Also provide the basis for the acceptable number of reactor trips in terms of the limiting component(s).
