NRC INTEGRATED INSPECTION REPORT NO. 50-261/96-12
Dear Mr. Hinnant
On November 16, 1996, the NRC completed an inspection at your H. B. Robinson
reactor facility. The enclosed report presents the results of that
inspection.
During the six-week period covered by this inspection period, your conduct of
activities at the H. B. Robinson facility was generally characterized by
safety-conscious operations, sound engineering and maintenance practices, and
careful radiological work controls. We are concerned, however, about three
violations of NRC requirements that were identified. In the first instance,
inadequate configuration controls for performing reactor coolant system
pressure isolation valve leakage testing resulted in a water hammer event in
the Safety Injection Cold Leg Injection lines. The second and third instances
involved inadequate Motor-Operated Valve testing and design controls as part
of your implementation of Generic Letter (GL) 89-10 "Safety-Related Motor
Operated Valve Testing and Surveillance." This inspection determined that you
had not satisfactorily completed implementation of GL 89-10.
These violations are cited in the enclosed Notice of Violation (Notice), and
the circumstances surrounding the violations are described in detail in the
enclosed report. Please note that you are required to respond to this letter
and should follow the instructions specified in the enclosed Notice when
preparing your response. The NRC will use your response, in part, to
determine whether further enforcement action is necessary to ensure compliance
with regulatory requirements.
