Gentlemen:

This refers to the Enforcement Conference held between Southern California

Edison Company and Region V management at the Region V office on May 3, 1991.

The matters discussed during this conference are described in the enclosed

meeting report. A copy of the conference agenda is also attached.

The purpose of this meeting was to discuss the significance of the misalign

ment of a Unit 1 containment spray flow limiter valve, S1-CRS-CV-518, which

occurred during the cycle refueling X outage. This misalignment was not

discovered until 22 months later, during the Cycle XI refueling outage. This

occurrence was the subject of Special NRC Inspection Report No. 50-206/91-11,

dated April 25, 1991.

Based on the results of our special inspection, as discussed during the

enforcement conference, it appears that one of your activities was not

conducted in full compliance with NRC requirements, as specified in the

enclosed Notice of Violation. Although we consider this violation to have

been a serious lapse in the proper control of safety systems, we view it as

another example of a larger issue for which enforcement action has already

been taken. That issue involved insufficient attention by your staff to the

thorough resolution of safety problems, as addressed by our January 4, 1991

Notice of Violation and Proposed Imposition of Civil Penalties for Unit 2.

In the Unit I case, a questioning attitude was not demonstrated in February

1989 when the redundant valve, CV-517, was found to be in an abnormal confi

guration. If a more inquiring assessment had been conducted at that time,

the misalignment of CV-518 would likely have been detected or prevented.

During this conference you stated that additional analysis had shown that

with valve CV-518 misaligned, sufficient containment spray flow would have

been provided, even if no operator actions were assumed. This conclusion was

subsequently documented in Licensee Event Report (LER)91-001, Revision 3.

You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice when preparing your response. In that you

have submitted a Licensee Event Report (LER 91-001, Revision 3) on this

subject, corrective actions already identified in that document may be

included by reference. After reviewing your response to this Notice and your

discussion in the LER, including your proposed corrective actions, and after

considering the results of future inspections, the NRC will determine whether

further NRC enforcement action is necessary to ensure compliance with NRC

regulatory requirements.

In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of

this letter, the enclosures, and your response will be placed in the NRC

Public Document Room.

The response directed by this letter and the accompanying Notice are not

subject to the clearance procedures of the Office of Management and Budget as

required by the Paperwork Reduction Act of 1980, PL 96-511.

Should you have any questions concerning this inspection, we will be pleased

to discuss them with you.
