Docket No. 50-206 SEP Topics: IV-2, Reactivity Control Systems VII-l.A, Isolation of Reactor Protection System from Non-Safety Systems San Onofre Nuclear Generating Station Unit 1 It is the purpose of this letter to provide information pertaining to the resolution of the subject SEP Topics. The information that follows is intended to be input to the Integrated Plant Safety Assessment for San Onofre Unit 1. SEP Topic IV-2, Reactivity Control Systems Our letter of May 7, 1984 indicated that we were evaluating the consequences of the remaining open items of this topic and provided our schedule for transmittal of the results. The evaluation was to demonstrate the acceptability of the rod motions that were determined credible single failure events. Accordingly, the following information is provided. The rod motions that could occur as a result of single failures are as defined below, with the corresponding discussion regarding the acceptability of the consequences. The single failure rod motions are:
- 1. Two banks of control rods may move simultaneously instead of one bank.
Response
This condition has been analyzed as presented in the San Onofre Unit 1 FSA (Ch. 7) and more recent Reload Safety Evaluations for Cycle 8. Cycle 8 reanalyses were performed to bound various tube plugging/sleeving conditions and various reactivity insertion A rates. The Uncontrolled RCCA Bank Withdrawal at Power and from a Subcritical Condition analyses bound the moving of two consecutive RCCA banks of the maximum combined worth. - 2. A cluster, group or bank of shutdown rods may not move when movement is commanded.
Response
Shutdown rods are positioned either completely in or fully withdrawn and are never used for control (Technical Specification 3.5.2). In all cases of single failures identified in the "Failure Modes and Effects Analysis for the San Onofre Nuclear Generating Station Unit 1 Rod Control Study" the shutdown and control rods were determined to be trippable. In order to accomplish their safety function, the shutdown rods need only be able to fall in order to trip the reactor.
- 3. A cluster, clusters, group, bank or banks of control rods may not move when movement is commanded.
Response
Immovability of a cluster of control rods may result in a misalignment of the cluster. This is bounded by the Static Misalignment analysis performed for each cycle and has shown that the DNB design basis is met. A stuck bank or banks is equivalent to manual rod control and is bounded by the safety analyses presented in the FSA or Cycle 8 Tubeplugging/Sleeving reanalyses, which assume manual rod control when it is limiting. Any overlap violation would be detected by alarms and is covered by Technical Specifications 3.5.2, "Insertion Limits", 3.5.3, "Misalignment" and 3.5.4, "Position Indication". Clusters and groups are also addressed in Technical Specifications 3.5.2, 3.5.3 and 3.5.4. As stated in response to Item 2 above, all rods that are immovable as a result of single failures were determined to be trippable.
- 4. A group of shutdown rods could move inadvertently.
Response
Inadvertent insertion of the shutdown banks would be detected and corrected as required by Technical Specifications 3.5.2, 3.5.3, and 3.5.4. Due to the fact that more than one group makes up a shutdown rod bank, withdrawal would be bounded by the RCCA Withdrawal transient analyses as discussed in the response to Item 1. The above discussed responses indicate that the consequences of the credible single failure rod motion events are acceptable for San Onofre Unit 1.
Mr. W. August 21, 1984 SEP Topic VII-1.A, Isolation of Reactor Protection System from Non-Safety Systems Our letter of March 30, 1984 provided you with the results of our further evaluation regarding the isolation of certain RPS circuits and their remote meters. Pursuant to subsequent discussions with the staff reviewer for this SEP Topic, the following supplemental information is provided. The functionality of pressurizer pressure and pressurizer level circuitry is assured by the independence of the three channels, two of which are required for a trip. The only area where the circuits are not completely physically separated is the remote recorder transfer switch. These switches are Westinghouse Electric Corporation Type W-2. By their design the potential for common mode failure exists only if there is a catastrophic failure of the cabinets in which they are mounted. Additional information to support this aspect of design is contained in Westinghouse product technical bulletins provided as Enclosure 1 and pictures of the switch installed at San Onofre Unit 1, one copy of which is provided as Enclosure 2 to this letter. Therefore, the functionality of these RPS circuits is provided by the channel independence. The steam to feedwater flow mismatch circuitry, while not isolated from the remote meters or the optimac computer that controls feedwater flow and steam generator level, was manufactured by the same company. The steam to feedwater flow mismatch circuitry, the metering equipment and the optimac computer were all provided by Hagen Controls. Although the metering equipment and the computer were not specified to be the same quality class as the steam to feedwater flow mismatch circuitry, they were subject to similar manufacturing quality controls and therefore can be assumed to have a similarly low failure rate. The design drawings for this equipment are provided as Enclosure 3 to this letter. Therefore, the physical independence and the low failure probability of the metering equipment and control computer provide assurance of the integrity and functionality of the steam to feedwater flow mismatch circuitry. The circuit analyses mentioned in our evaluation of the startup rate neutron monitor and the high flux level circuitry are provided as Enclosure 4 to this letter. This analysis provides evidence that presently installed equipment will function to provide adequate isolation for these RPS circuits. It is compact, yet of sufficient size to provide long, trouble-free service when applied within the specified ratings. Design flexibility of internal parts and con tacting, plus external jumpering of the contact studs affords almost limitless switching combinations for relatively few standard components. The design flexibility is provided in the basic switch styles. A complete switch is made up of a basic switch, one of several styled handles, external jumper, (when required) and a nameplate. TheW-2 switch may be ordered as a single Type W-2 style unit where such a style meets a specific application. Where a single listed style does not meet a specific application, the desired switch may be ordered by specifying the required styled components. As separate items or by completing Contact Tabulation Form 34219. The materials used in the manufacture of the Type W-2 Switch make it virtually inherently fungus proof. ENCLOSURE 4 SEP TOPIC VII-l.A REACTOR PROTECTION SYSTEM SCE SONGS-1 During the development of the recommended licensing position for Systematic Evaluation Project (SEP) Topic VII-l.A, Impell stated that resistor buffer isolation existed between the recorders of the Start Up Rate Neutron Monitor and High Flux Level Systems. This report now documents the actual resistor configuration for these isolation buffers. The following resistor configuration currently exists on these Reactor Protection Systems: RPS Analog RPS Analog sign 1 sig al 38.3 K 38.3 K 49.9 K 49.9 K Recorder ERecorder HIGH FLUX LEVEL SYSTEM START UP RATE NEUTRON MONITOR SYSTEM From these circuit diagrams it can be seen that these non-safety recorders are isolated from their respective RPS analog signals by resistor buffers in excess of 1900011.
