Exekiit Braidwood Station Unit 1 AiRi2 ISI Outage Report 5.0 REPORT OF CONTAINMENT DEGRADATION The NRC amended 1 OCFR 50.55a to incorporate by reference the 1992 Edition with the 1992 Addenda of Subsection IWE (Requirements for Class MC and Metallic Liners of Class CC Components of Light-Water Cooled Power Plants) and Subsection IWL (Requirements for Class CC Components of Light-Water Cooled Power Plants) of ASME Section XI, Division 1, with specified modifications and limitations. The following section is included in the Inservice Inspection Summary report required by IWA-6000 of ASME Section XI to meet the reporting conditions specified in 1 OCFR 50.55a(b)(2)(viii)(A)-(E) and 10CFR 50.55a(b)(2)(ix)(A)-(E).

5.1 Al Ri 2 Containment Concrete Examinations (IWL)

Unit 1 and Unit 2 IWL tendon and concrete examinations under the 20th year post tensioning surveillance began in March of 2006, and are scheduled to be complete in September of 2006 (prior to the A2R12 refuel outage). Concrete examinations for both units were ongoing at the time this report was prepared. Final reporting of anydegraded conditions of Unit 1 containment concrete described in 10 CFR 50.55a(b)(2)(viii)(E) will be included in the A1R13 (FaIl 2007) IWA-6000 report. Unit 2 IWL tendon and concrete reportable conditions [10 CFR 5U.55a(b)(2)(viii)(D) and 10 CFR 50.55a(b)(2)(viii)(E)] will be provided in the A2R12 (Fall 2006) IWA-6000 report.

At the time this report was prepared, Unit 1 tendon inspections had been completed. The presence of free water was the only 10 CFR 50.55a(b)(2)(viii)(D) reportable condition identified.

The following table documents identified conditions as required by 10 CFR 50.55a(b)(2)(viii)(D)( 1) for sampled sheathing filler grease containing chemically combined water exceeding 10 percent byweight or the presence of free water.

Braidwood Station Unit 1 AiRi2 ISI Outage Report 5.2 Al Ri 2 Containment Examinations (IWE)

A Section XI IWE augmented examination, which includes a VT-i surface examination and ultrasonic examination per the requirements of ASME Section Xl, Table IWE-2500-l, Examination Category E-C of the Class CC liner was scheduled for AiRi2. This was the second scheduled IWE augmented examination of the Unit 1 liner.

A Section Xl IWE General Visual Examination per requirements of Table IWE-2500-1, Examinations Category E-A was also scheduled for A1A12.

Exelon Procedure EA-AA-330-007 was used to perform the augmented and general visual examinations.

Exam Data Form from Exelon Procedure ER-AA-335-U1 8 was generated to characterize the extent of indications on the Class CC liner as a result of VT-i surface exam and general visual examination.

The IWE VT-i examination is summarized in the following sections:

5.2.1 Description of Degradation Type and Estimated Extent of Degradation Nineteen augmented areas were examined. All areas had insignificant changes and were essentially unchanged. The maximum metal reduction found was identical to the Al RiO reduction of 5/64 deep. This was at location #3, which remained unchanged. There was one location, #19.5, which had a measured pit depth change from 2/64 to 3/64 since last augmented exam in A1A1U. This change in pit depth measurement may be contributed to difference in the conditions it was measured during Al RiO and AlRi2 outages. The pit depth for area #19.5 measured in Al RiO was with coating off. The AlAi2 pit depth was measured with coating in place. The As-Found condition of liner coating at location

  1. 19.5 was excellent for AlRi 2. The entire location #19.5 was still coated from Al Rb.

There was no sign of any active corrosion. The moisture barrier was in tact and Cerablanketwas dry when removed. The pit is on the I.D. of liner. The four U.T. data points at location #19.5 exceeded.25 (nominal liner thickness). This augmented area will be re-examined in the next period.

The nineteen augmented areas were ultrasonically examined and the following are the results:

All augmented areas were ultrasonically examined. The IWE-35l2-3 ultrasonic examination requirement of not exceeding 10% nominal containment wall thickness was met at all augmented areas. The liner thickness measurements varied from 0.248 to 0.265. Nominal liner thickness is 0.25. No further evaluation is required for the ultrasonic results.

The above-described conditions of the accessible areas (where the moisture barrier and Cerablanket were removed in A1A12) do not indicate the presence of or result in degradation to the inaccessible areas that are located below these accessible areas.

5.2.2 Conditions Leading to Degradation The degradation to the liner was a result of moisture barrier aging. This was determined during Al R08 outage.

The moisture barrier degradation combined with impingement of water as a result of fan cooler condensate leakage established the conditions for liner degradation. Water had a path to flow between the moisture barrier and the liner. Water was entrapped below the Tremco epoxy sealant into the Cerablanket material and above the adhesive used to anchor the compressible material in the 2-inch annular space Page 5-3

Braidwood Station Unit 1 A1A12 ISI Outage Report between the basemat and the liner. The surface of the liner remained wetted, and with the presence of oxygen, the liner began to degrade.

The localized maximum wastage of material was measured to be 5/64 of an inch, which is 32% of the liners nominal thickness of 1/4 inch.

It is apparent that the corrective actions that were performed during A1RO8 and AiRlO have corrected the problem of any further liner metal reduction.

The examined areas were dry and there was no active corrosion found beneath the removed moisture barrier.

The corrective actions that took place during Al R08 were as follows:

1)

Remove the entire moisture barrier and 3 of Cerablanket below it.

2)

Aecoat the exposed liner below the moisture barrier with Service Level 1 qualified coating.

3)

Install new dry Cerablanket and moisture barrier.

52.3 Evaluation of Each Area and Results of Evaluation Calculation 5.2.6-BRW-98-i090 and calculation BYR98-O04 had addressed localized defects in the liner plate membrane in the form of gouges created during the concrete removal activities for the Steam Generator Replacement (SGA) projects at both Byron and Braidwood Stations. The calculations evaluate the effect of those defects based on criterion from calculation 5.2.6.1.4 for plate design in conjunction with conservative plate behavior characteristics. These calculations conclude that gouge depths as great as 1/8 inch are acceptable. The additional buckling strain introduced by the reduced thickness of the plate, when combined with the non-thickness dependent strains, results in a total strain of approximately 0.00834 in/in, which is below the code allowable strain of 0.01400 in/in.

Since the reduction in the liner plate thickness evaluated in calculation 5.2.6-BRW 1090 is not source dependent, i.e. gouges, nicks, etc., localized reduction in the liner plate thickness due to corrosion and degradation can also be evaluated using the same methodology and conclusions. Therefore, reductions in liner plate thickness as great as 1/8 inch due to corrosion and degradation are also acceptable.

The defects in the liner plate recorded in the inspection report have been reviewed and evaluated with respect to this acceptance criterion. The defects identified are all bounded by this 1/8-inch plate thickness reduction limit and are, therefore, acceptable as-is.

Local defects in the Braidwood containment liner plates not exceeding 1/8 inch in depth are acceptable. The reduction of plate thickness in the areas of degradation is less than 1/8 inch.

The maximum of 1/8 inch reduced thickness of the liner plate has been shown by analysis to satisfy the requirements of the design specifications.

Therefore, liner reduction is acceptable for continued service without removal or repair in accordance with IWE-3122.4.

The corrective actions that took place during A1R1O were the same as A1AO8 except they were performed in the augmented areas only.

The exposed liner area directly below the moisture barrier approximately 4 in width, where the augmented exam was performed was surface prepared for Level I coatings application.

A Service Level 1 qualified coating was applied to the portion of the liner below the moisture barrier that was exposed for augmented exam per WO 745607. New dry Cerablanket material was installed below the MB.

The MB was restored per WO 784477.

The entire moisture barrier was removed during AlR08 outage. This AlAl2 exam is an augmented exam of areas found exceeding 10% metal reduction during AlROB and Ai A1O. The exposed liner areas had an in tactdymeric moisture barrier and the removed Cerablanket was dry. The liner was also dry and there was no evidence of active liner corrosion taking place.

Since the examination areas were dry, the maximum metal reduction was not exceeded. There was no active corrosion taking place and there were no indications of degradation in any other inaccessible areas. Approximately 80% of the entire moisture barrier was also VT-3 examined with no indications identified that would permit any water intrusion to take place.

All previously identified augmented areas were examined.

There are no additional examinations in accordance with IWE-243O required.

All the augmented areas shall be re-examined over the third and final augmented period except for augmented areas that had some change or were initially documented during Al RiO outage. These areas will be re-examined over the next 2 inspection periods until the areas examined remain essentially unchanged for three (3) consecutive inspections periods.

Description of Worlo Existing pressurizer heater sleeve and adjacent welded couplings at Penetration #52 were removed from service and replaced by a welded plug in accordance with EC 360630 after through-wall leak was discovered at existing heatersleeve. The replacement was performed under the AREVA Framatome ANP program. Liquid penetrant examinations were performed on both the initial and final weld passes. The removed heater sleeve and adjacent couplings were sent out for analysis to perform a root cause evaluation of the failed heater sleeve.
