INFORMATION REQUEST TO SUPPORT THE 95002 INSPECTION

This letter is to provide you with advanced information regarding materials needed to support

our Inspection Procedure 95002 inspection which is scheduled to begin on September 12,

2005, at your facility. We will review the requested material and identify those areas where

more detailed information will be needed.

In accordance with 10 CFR 2.390 of the NRC's "Rules of Practice," a copy of this letter and its

enclosure will be made available electronically for public inspection in the NRC Public

Document Room or from the Publicly Available Records (PARS) component of NRCs

Requested Information to Support IP 95002 Inspection

The NRC IP 95002 team leader will conduct an on-site receipt of the requested information with

you on or about August 16, 2005. Please provide the following information or documents.

{Note: Region IV NRC staff agrees with Palo Verde Nuclear Generating Stations staff desires

to minimize hard copies of requested information. Therefore, electronic files of the requested

materials are preferred.}

1. An index file of all administrative procedures associated with corrective actions, cause

assessments, extent of condition evaluations, effectiveness reviews, self assessments,

and issue tracking and trending.

2. An index file of all administrative procedures associated with engineering design control,

the 10 CFR 50.59 process, engineering evaluations, and operability evaluations.

3. Administrative procedures governing the identification and resolution of problems.

4. Administrative procedures governing root cause and apparent cause evaluations.

5. Administrative procedures governing extent of condition reviews.

6. Any administrative engineering procedures governing the translation or verification of

Design Basis Document values, including setpoints, into mitigating systems Technical

Specifications or Surveillance Requirements.

7. Any previous revisions of items 3, 4, 5, and 6 for the last 2 years.

8. All corrective action documents, root or apparent cause evaluations, and assessments

associated with the violations specified in NRC special inspection report 2004-014

(EA-05-051).

9. Any Quality Assurance audits associated with attributes of the violations specified in

NRC special inspection report 2004-014 (EA-05-051).

10. A list of all significant conditions adverse to quality, with a corrective action program

document reference, identified for mitigating systems since January 1, 2004.

11. A list of all conditions adverse to quality, with a corrective action program document

reference, identified for mitigating systems since January 1, 2004.

12. A list of all mitigating systems changes, modifications, or procedure changes that

required a 10 CFR 50.59 evaluation since January 1, 2004.

13. All 10 CFR 50.59 required screenings or evaluations reperformed as a result of the NRC

finding or your self assessments.

14. A list of the corrective action system backlog.

15. A list of the maintenance backlog associated with mitigating systems.

Also, either provide electronically, or have available on site upon the arrival of the inspectors,

the Palo Verde Updated Final Safety Analysis Report, Technical Specifications, and Technical

Specification Bases for Units 1, 2 and 3.
