The information contained in the January 9, 2003 letter relates to a request for license amendment and should therefore have included an oath or affirmation consistent with the Office of Nuclear Reactor Regulation (NRR) Office Letter LIC-101 Revision 2, "License Amendment Review Procedures." An affidavit is attached attesting the accuracy of our January 9, 2003 letter.
