~Findin As required by 10CFR 50, Appendix 8, Criterion VI measures shall be established to control the issuance of documents.

Florida Power and Light Company's accepted Quality Assurance Program (FPLTQAR 1-76A), Section

6. 1, requires that procedures shall provide for controlling obsolete documents to preclude their use.

This requirement is implemented by Quality Procedure

6. 1 which states that superseded controlled documents shall be destroyed, segregated or clearly identified as superseded.

Contrary to the above, revision 17 of Operating Procedure

0030122, "Reactor Startup" was used to control activities in performing startup number 117 on May 15, 1980.

Revision 18 of that procedure was approved on February 28, 1980 and had been in use since May 7, 1980.

(No other startups were performed between the latter two dates.)

In addition, instructions were provided to all licensed operators regarding this subject.
