APPENDIX A NOTICE OF VIOLATION Florida Power and Light Company St. Lucie Unit 1
License No. DPR-67 Based on the NRC inspection June 24 to July 2, 1980, certain of your activities Were apparently not conducted in full compliance with NRC requirements as indicated below.
These items have been categorized as described in correspondence to you dated December 31, 1974.
As required by 10CFR 50, Appendix B, Criterion VI measures shall be established to control the issuance of documents.
Florida Power and Light Company's accepted Quality Assurance Program (FPLTQAR 1-76A), Section 6.1, requires that procedures shall provide for controlling obsolete documents to preclude their use.
This requirement is implemented by Quality Procedure 6.1 which states that superseded controlled documents shall be destroyed, segregated or clearly identified as superseded.
Contrary to the above, revision 17 of Operating Procedure
- 0030122, "Reactor Startup" was used to control activities in performing startup number 117
.on May 15, 1980.
Revision 18 of that procedure was approved on February 28, 1980 and had been in use since May 7, 1980.
(No other startups were performed between the latter two dates.)
This is an infraction.
