Thank you for your letter of January 2, 1991, in response to our Notice of Violation and Inspection Report No. 50-529/90-45, dated December 4, 1990, informing us of the steps you have taken to correct the items which we brought to your attention.
Although your response to this Notice of Violation recognizes procedural inadequacy as the reason for the violation, your corrective actions do not appear to. address the reasons why these procedures were deficient.
The performance of a more detailed root.cause evaluation appears appropriate.
Recent events such as the over-dilution of Unit 1 while at power, which were partly caused by procedural inadequacies, suggest a need to strengthen your procedure development processes.
It appears that your maintenance procedure development process may benefit from a similar type of review you gave the operations procedure development process following the Unit 1 dilution event.
We'ould encourage you to maint'ain an appropriate depth of review of such issues so that under lying program weaknesses can be fully brought to light and addressed.
This issue will be reviewed during a future inspection.
