Results Of the five areas inspected, no items of noncompliance or deviation were identi-fied.

Exit Interviews The weekly inspection scope and findings were summarized with the project manager on May 2, May 19, 23 and 30, 1980. The resident inspector attended an exit interview conducted on May 23, 1980 by RII metallurgical engineer P. K. Van Doorn scoping his welding inspection.

-2-r The appended n.cmorandum dated May 3,1980 from W. T. Quinn to Files, gives chronology of the construction delay to that date; a status of required a

activities, outstanding NCR's and QCIRs; a listing of stored items; copies of the concrete pour schedule drawings; and drawings of temporary mechanical and civil features to suoport the shutdown preparation and maintenance.

These documents appear to be adequate for protection and maintenance of the shutdown facilities.

(Closed) IFI 520/80-06-01 Methods for Maintenance of Protective Atmosphere in RPV X-19 During Horizontal Storage.

This IFI has been superseded by Deficiency 520/80-08-04 Failure to Follow Procedures for Storage of RPV.

Unresolved Items Unresolved items were not identified during this inspection.

Independent Inspection Effort During this report period, licensee representatives, the NSS supplier representatives, ASME authorized inspectors and individual craftsmen ini-tiated discussions with the inspector with questions concerning QA auditing, 50.55(e) rer ting levels and timing, stored equipment maintenance, ASME Code requi ements on welder qualifications and field work conditions which could negatively impact quality.

The inspector was kept informed concerning reorganization of the Construction engineering groupings.

STRIDE and BOP QC and site project engineering now report to a single con-struction engineer; W. T. Quinn, under whom position changes, responsibilities, authorities and personnel reassignments were to become effective June 2,1980.

The effectivity of these changes on quality assurance will be monitored by the inspector.

(Closed) 518, 519, 51520, 521/79-30-01 Unacceptable Bending of Welded Stud Anchors On Embedded Plates (NCR ENP-A-064)

The licensee has completed experiments dete rmining the degree (45 degrees) of bending of studs permissable without reduction of effec-tiveness. No degredations of design capacity of the weld anchor studs has occurred.

In March 1980, TVA Engineering design issued Criteria to TVA Construc-tion establishing detailed guidelines for maximum bending of welded stud anchors for each type of embedment.

Implementation of these criteria are being monitored by TVA QC and QA (auditing).

This resolution is acceptable.

(0 pen) 518, 519, 520, 521/80-10-01 Defects In Rosemont Trip / Calibration Units General Electric has initially identified 54 items of equipment for Hartsville which may contain defective pressure transmitters due to defects in Rosemont Model 510 DU trip / calibration units.

Corrective Actions:

GE has undertaken an investigation to determine the locat ion of all equipmer.t which has one r more of the subject defective components installed.

Rosemont will replace the defective switches in the affected trip units and the defective capacitors in the affected transmitter units that have been shipped to GE, including those shipped on to utilities by GE.

(0 pen) 518, 519, 520, 521/80-10-02 Design Deficiency of T-Head Section of Anchor / Darling Gate Valves (NCR - 6)

TVA's Architect Engineer, C. F. Braun, Co.,

rejected the vendor's proposed modification to the valves under which 50.55(e) items 518, 519, 520, 521/79-27-07 had been closed out.

Deficient valve disks are to be replaced by new disks cast to the proper configuration.

(0 pen) 518, 519, 520, 521/80-10-03 Failure to Include Requirements for Environmental Qualification of Mechanical Components in STRIDE Procure-ment Specifications (NCR CFB-8).

Engineers failed to include all applicable requirements in specifica-tions.

Lakes, Dams and Canals - Spray Ponds for Plants A & B Earth compaction and structural concrete placement were under way during this report period.

The work criteria are described in TVA Construction Specification N 6C - 875, Revision 12 dated 3/28/80, " Earth and Rock Foundations and Fills" and by notes on the construction drawings.

Inspection and testing are guided by Civil Quality Control Instruction C-102, Revision 7, dated 8/30/79, " Foundation and Fill, Earthfill Control Testing".

I Concrete installation is controlled by C. F. Braun Specification 300-01 CONCRETE.

Inspection and testing is controlled by TVA Civil procedures QCI-C-201 to 218.

The inspector inspected earth fill and compaction and compaction testing in the area south of spray ponds A-1 and B-2 and adjacent to the proposed rock fill dike. Calculations for several compaction tests were reviewed at the soils laboratory.

Concrete placement B2KESWPS38, totaling 156 cubic yards was inspected and subsequent curing actions were inspected. The placement was for an inter-mediate level slab in ESW Pump Station B-2.

No noncompliances or deviations were identified during inspection of the earth fill and concrete placement.

Extended Construction Delay - Plant B - Review of QA Pacedures i

During this report period the licensee issued procedure HNP-SOP-58 Periodic Inspection of the Delayed Portion of Hartsville Nuclear Plant B, which incorporates a listing of work items deemed necessary for the protection and maintenance of the incomplete facilities and a listing of open QCIR*=

and NCR's.. Construction drawings are provided for civil an/ mechanical j

work required to support completion of shutdown and provide for maintenance during the extended construction delay, now estimated to last eight more

years.

These documents, supported by standard procedure C. E. P. 13.02 Storage and

Preservation of Materials, Components and Systems, were reviewed and judged to be adequate for quality assurance.

l Unit A-1 Vent Wall Concrete A follow on inspection of-work on Unit A-1 Vent Wall was made. The inspector

inspected the placement of concrete filler in the first 120 degrees section

of the A-1 vent wall. The number and complexities of the cross members between the inner and outer steel walls and the height of the wall required unusual c.ontrol of the concrete mix, its placement by extra long tremies and chutes, vibration, and the finishing of the top surface.

Placement criteria were imposed by C. F. Braun specification 300-01 STRIDE Concrete. Inspection was guided by QCI-C-201.

Status of OI & E Bulletins The inspector determined that the licensee receive * and reviewed for appro-priate action the following bulletins:

(Closed) 80-02 Inadequate Quality Assurance For Nuclear Supplied Equipment.

Marvin Engineering feedwater spargers or thermal sleeves have not been purchased for, or installed in liartsville units.

(Closed) 80-07, Supplement No.1 BWR Jet Pump Assembly Failure No actions on this bulletin by a construction permittee is appropriate or required.

(Closed) 80-10 Ccutamination of Nonradioactive System and Resulting Potential For Unmonitored, Uncontrolled Release of Radioactivity to Environment.

The Bulletin was for information only to plants under construction where no fuel has arrived on site.
