SUMMARY Inspection on September 2-27, 1980 Areas Inspected This routine announced inspection involved 52 inspector-hours onsite by the resident inspector in the areas of: earth compaction; rock fill and concrete for spray ponds; structural concrete in Plant A power block buildings; A-2 reactor pressure vessel handling; review of open items; and independent in-spection effort.

Results Of the five areas inspected, no items of noncompliance or deviations were iden-tified.

Exit Interview The inspection scope and findings were summarized with the Acting Project Manager on September 5,1980 and with the Project Manager on September 22, and 26,1980.

Licensee Action on Previous Inspection Findings (Open) Infraction, 518, 520/80-18-01, Failure to Take Timely Corrective Action on Drawing Control Findings The_ licensee by Monday, September 29, 1980, had verified that all drawing prints in the Sands of the using departments were the most recent available and that obsolete prints, or the title blocks thereof, had been retrieved.

There are still unresolved questions on the currency of engineering change notices (ECNs) and field change notices (FCNs). However, interim management controls appear to be adequate to prevent recurrence of the problems cited involving drawing control.

Unresolved items were not identified during this inspection.

5.

Independent Inspection Effort The inspector made routine surveillance tours during which the status of project work was noted, and construction activities were inspected on nuclear safety related structures, systems and facilities.

Inspection effort was concentrated on Plant A areas.

Follow on inspections of structural concrete were made in the Plant A power block structures. Forming, installation of rebar and embedments, placements and post placement curing were inspected as work progressed for the A-2 control building, the A-1 diesel building footing, and the A-1 and A-2 shield building walls. At the end of the period, preparations were inspected for the 328 cubic yard placement AIR-13b at the '

s of A-1 containment between the RPV pedestal and the weir wall. Ade

.e inspection by TVA QC for these operations was verified. Concrete it.tallation activities are controlled by C. F. Braun Specification 300-01, Revision 8 " CONCRETE".

No items of noncompliance or deviations were identified.

Licensee Identified 50.55(e) Items a.

Previously Identified Items (0 pen) 518, 520/80-05-03 RPV Shield Wall Weld Deficiencies (NCR's HNP-A-087,HNP-A-108,HNP-A-109,SX-19).

TVA's investigation of weld defects in the A-1 shield wall revealed a similar defect in the Unit A-2 structure.

This 50.55(e) item is modified to include Unit A-2, Docket No. 50-520. Additionally, TVA has found what appears to be slugged welds on tier 3 of the Unit A-1 shield wall. The inspector has inspected two sample removals which reveal apparent " slugging."

Licensee's final report on this matter is due by December 24, 1980.

(0 pen) 518/80-21-01 Deficient Six Inch Borg-Warner Gate Valves (NCR 23)

Two valves were received in which the gate could separate from the guide due to a design error according to a report by Borg-Warner, the manufa cturer.

TVA anticipated that the valves will be modified by March 1, 1981.

-3-r Design deficiencies were reported by General Electric involving improper separation of power supplies in the NSPS Panels.

The panels for Hartsville Units A-1, A-2, and B-1 had been shipped to the site and the panel for Unit B-2 has been fabricated but not shipped.

The received panels were returned to GE, San Jose, for redesign and rework to the corrected design.

TVA's final report on tt 's item is expected by November 19, 1980.

Reactor Vessel Installation - Observation of Work and Work Activities -

Unit A-2 (Storage of RPV Adjacent to A-2 Containment)

RPV X-20 (B13-D002) was received by barge oa September 17, 1980, unloaded and transported to the lifting position of the temporary gantry crane at the north side of A-2 Containment. The inspector inspected the subsequent work of rigging, final positioning of the vessel for lifting, lifting it to the vertical position, removal of the shipping =Pirt, and placing of the upright vessel on a borrowed upper pedestal section (3-1) supported by oak cribbing resting on a concrete pad.

Prior to lifting, the vessel was cleared of inert gas, entered and inspected. Ventilation was started with filtered air.

These operations were performed under STRIDE Work Packagc D280-M2 " Initial Set of Reactor Pressure Vessel".

The inspector inspected and photographed the control rod guide tubes and instrumentation tubing exposed by removal of the shipping skirt. There was no apparent distortion or damage.

In the operations inspected, no noncompliances or deviations were identified.

Lakes, Dam, and Canals Observation of Work and Work Activities (Spray

-

Ponds for All Units)

Follow on inspections were made of earth fill placement and compaction, installation of filters and shot rock for the rock dike, and concreting operations on the four essential service water pumping stations for the spray ponds.

The inspector again verified that earth work and rock dike placement were being performed in accordance with the requirements of TVA Specification NGC-875, R12 " Earth and Rock Foundations and Fills" and that pump station concrete operations conformed to the requirements of the controlling C. F.

Braun STRIDE Specification 300-01 " CONCRETE".

No noncompliances or operations de viations were identified.

The licensee had curtailed work for spray ponds B-1 and B-2, except that necessary to prevent erosion and provide other safeguards for the earth work and rock dike for spray ponds A-1 and A-2.

Also, construction of B-1

-4-i ESW pump station was halted and a wood frame structure was built over the structure to protect exposed reinforcing steel and embedments.

Sidewall concrete on B-2 ESW pump station is proceeding, but only a temporary roof will cover the structure. This curtailment of work for the Plant B spray ponds is in addition to previously announced plans to defer work on Plant B.

TVA plans to report the extent of work curtailment in the Final Safety Analysis Report (FSAR).

9.

Status of IE Circulars The inspector determined that the licensee has received and assigned for review the following Circular, which is closed for record.

(Closed) 80-21, Regulation of Refueling Crews Not applicable to Hartsville at this item. First refueling will occur in approximately seven years from now.
