Exit Interview The inspection scope and. findings were summarized by NRC inspector N. Economos on November 14, 1980 with those persons indicated in Paragraph 1 above.

Licensee Action on Previous Inspection Findings Not inspected.

Unresolved Items Unresolved items were not identified during this inspection.

5.

Licensee Identified Item The inspector examined the licensee's disposition of nonconformance report HNPA-114.

This nonconformance report was generated as a result of the omission of reinforcing steel from the auxiliary building zone 1 slab at elevation +11-0".

The missing steel is in the area of electrical equipment foundation pads and the designer has indicated that these foundation pads are over-designed and that the steel is not necessary.

However, the nonconforming condition is considered significant in that the failure of responsible ironworkers, and project and civil engineers to identify the missing steel during the final inspection signoff prior to pouring the zone 1 slab represents a breakdown in the QA program.

Procedure CEP 16.03, paragraph 2.2A requires that the cause of the condition shall be identified and documented and that the documented corrective action shall address' measures to prevent recurrence of same or similar conditions.

The cause of the omission of reinforcing steel from the zone 1 slab is not documented on nonconformance report HNPA-114.

The recommended corrective action on HNPA-114 states that ironworkers, project and QC engineers shall be instructed to be more observant of

what is added to drawings and engineering change notices (ECN's).

Examination of pertinent ECN's and drawings by the NRC inspector and discussions with responsible engineers indicated that the cause of the NCR is that ECN's were not clear in that required changes were not called out and that a large number of ECN's are being generated before drawings are updated.

Failure to identify the cause of the nonconforming condition and take adequate corrective action was identified to the licensee as violation item number 518/80-24-01 and 520/80-24-01.
