Inspection on January 6 through March 17, 1983 (Report No. 50-461/83-04(DPRP))
Areas Inspected: Routine safety inspection performed by the Senior Resident Inspector of construction activities including material laydown and storage areas, program control, general welding activities, and numerous stop work
recovery activities. The inspection involved a total of 157 inspector-hours onsite by one NRC inspector, including 12 inspector-hours onsite during offshifts.
Results: Of the areas inspected no items of noncompliance or deviations were identified.

Functional or Program Areas Inspected a.
Site Surveillance Tours At Periodic intervals during the report period, surveillance tours of areas of the site were performed. These surveillances were intended to assess:
the cleanliness of the site; the storage and maintenance conditions of equipment and material being used in site construction; the potential for fire or other hazards which might have a deleterious effect on personnel or equipment; and to witness construction activities in progress.
In general, the storage and maintenance of safety related material and equipment appeared acceptable throughout the laydown areas and the Power Block.
No items of noncompliance or deviations were identified.
b.
Program Control At periodic intervals, the SRI reviewed Nonconformance Reports (NCRs), Discrepancy Reports (DRs), S&L Specifications Revisions, BA Audit Reports, Construction Procedure Revisions, IP Quality Procedure Revisions, Trend Analysis, IP Surveillance Reports, Findings, Audits, and plant problems identified by Corrective Action Requests (CARS)
and other means. Numerous informal comments were discussed with the licensee and contractor. The SRI attended numerous status and/or problem discussion meetings with IP and BA.
The SRI reviewed and approved individual Stop Work Order (SWO)
exceptions and Partial Release Plans as allowed by NRC Confirmatory Action Letters (CAls).
No items o'. noncompliance or deviations were identified.
c.
Potential Problem Identification The following item was forwarded to the licensee for information and
" follow-through" action. The item consists of generic problem infor-mation obtained by the NRC from other power reactors. The NRC inspector explained that the information may identify a potential
,
l problem of a similar nature at Clinton. The licensee is requested to investigate the problem, determine its applicability, and denote

j action to be taken.
l Recent NRC inspections have uncovered apparent problems with Midland-Ross "superstrut" material used for Class IE cable tray and conduit support at some Western U. S. power plants. Details of the apparent quality assurance breakdowns were provided to the licensee, Illinois Power, on January 21, 1983.

d.
Welding Activities (1) During the reporting period, the inspector observed general welding activities associated with electrical hangers and structural attachments in the Power Block. For the welding examined one or more of the following activities was observed.
Protection of nearby electrical cable was satisfactory.
Electrode control and handling was satisfactory. The welding areas were free of contaminants. The welders were certified as qualified for the thickness and the process specified.
Technique and weld results appeared to be satisfactory.
No items of noncompliance or deviation were identified.
(2) While performing a surveillance of a weld repair operation on an In-vessel Fuel Rack, the resident inspector extended the surveillance to include a visual inspection of existing aluminum fillet welds on the rack structure. Eight potential discontinuities were detected in fillet welds on one side of the rack. Types of discontinuities were incomplete fusion and cracks. The in-vessel fuel rack is one of two pieces of re-fueling equipment supplied by General Electric Company. The discontinuity information was forwarded to IP quality assurance for action.
Subsequently, a complete reinspection of both racks has been performed resulting in two cummulative noncon-fo rmances.
Corrective action is in progress and will be monitored.
This is a continuation of unresolved item 82-04-02.
3.
Meetings and Site Visits a.
A systematic Licensee Assessment Program (SALP) presentation was given in Region III by NRC on March 17, 1983. Attending to receive their assessment of yearly performance were management representa-tives of Illinois Power and Baldwin Associates. This assessment is detailed in report 50-461/82-21.
b.
On March 17, 1983, there was a meeting in Region III with NRC and Illinois Power management to discuss the status of their Nuclear Power Improvement Program. Details are provided in report 50-461/83-06.

Stop Work Chronology A summation of past Stop Work and Recovery Program events have been listed in NRC Reports 50-461/82-05, -08, -14, and -24.
For this general reporting period the following events have taken place:
a.
On November 18, 1982, Illinois Power embarked on an approved partial release work effort of Stop Work 016 Electrical Conduit. Commencement of Phases I through III, a controlled demonstration of work effort,

was approved by.the NRC Region III. This effort remains underway and is nearing completion.
b.
On December 15, 1982, NRC Region III concurred in the release of Phase II of Electrical Stop Work 007. This limited resumption of inspection and remedial electrical tray and connection work remains in progress.
c.
The results of the Stop Work 019 Containment Structural Steel, 2A-1 reinspection effort was reviewed and approved by NRC. Subsequently, on December 17, 1982, Region III approved reinspection phase 2A-Z, the remaining friction and slip connections in Containment.
Baldwin Associates is continuing with this effort at this time.
d.
On January 17, 1983, NRC Region III acknowledged receipt of Illinois Power's detail plan for disposition of the Drywell Refueling Bellows.
The NRC Region III reviewed the plan and concurred with the continued action on the partial release of Stop Work Order 010.
e.
On March 9, 1983, Illinois Power submitted their Record Verification and Overinspection plans to NRC Region III for review and concurrence.
A partial trial run of the Records Verification Program was approved on March 11, 1983 while the remainder of the program is undergoing a continuing review.
The ten original Stop Works (SWO) remain in effect and the three Confirmatory Action Letters (CAL) remain imposed.
5.
Exit Meetings The inspector met with IP representatives (noted in Paragraph 1) through--
out the inspection period and summarized the scope and findings of the inspections performed.
