St.LucieUnit2DocketNos.50-389ReplytoNoticeofViolationInsectionReort92-07RESPONSE(A)1~2~ThereasonfortheviolationwaspersonnelerroronthepartofMaintenanceSupervision.SupervisiondidnotverifytheproceduralrequirementsforTestGaugeCalibrationaccuracyweremet.Afactorcontributingtothispersonnelerrorwasthattherequirementsforgaugeaccuracywerenotspecifiedinallapplicablesectionsofthemaintenanceprocedure.CorrectiveStesTakenandResultsAchieved3~OnApril21,1992,MainSteamSafetyValvetestingwasstoppedandgaugecalibrationdatasheetswerereviewed.Gaugef201wasdeterminednottomeetthe.54accuracyrequirementandwasreplacedwithanewgaugemeetingthe.54accuracystandard.TheMainSteamSafetieswhichhadbeentestedwithGaugef201wereretestedusingthenewgaugeanddeterminednottorequireadjustment.Allpersonnelinvolvedweremadeawareoftheinfractionandtheneedforproceduralcompliance.CorrectiveStestoAvoidFurtherViolationsa)AmemowasissuedtoallProductionSupervisorsandForementostresstheimportanceofproceduralcompliance.4~b)DisciplinaryactionwasadministeredtotheMaintenanceSupervisioninvolvedinthisevent.c)MaintenanceprocedureGMP-0705willberevisedtospecifythegageaccuracyrequirementsinallapplicablesectionsoftheprocedureandincludegaugeaccuracyrequirementsaspartoftheQCholdpointtoverifyM&TEofpressuregauges.d)MechanicalMaintenancewillreviewallsafetyandreliefvalvetestprocedurestoensuretestgaugeranges,type,andaccuracyrequirementsareadequatelyspecified.FullcompliancewasachievedonApril21,1992,uponcompletionofcorrectiveactioninparagraph2.Maintenanceprocedurereviews/revisionswillbecompletedbyAugust21,199 Re:St.LucieUnit2DocketNos.50-389ReplytoNoticeofViolationInsectionReort92-07RESPONSE(B)~1.ThereasonfortheviolationwaspersonnelerrorwhichresultedintheinadvertentcappingofthehighpressuresensinglinetothecontainmentpressuretransmitterPT-07-2C.2~CorrectiveStesTakenandResultsAchieveda)ThecapattachedtothesensinglinewasremovedonMay7,1992restoringtheoperabilityoftheinstrumentchannel.b)Anexaminationofmaintenanceandoperatinghistorywasperformedtodeterminetheperiodofinoperabilityandtheworkauthorizedonthisinstrument.Documentedevidenceindicatesthatnorelatedworkwasauthorizedonthisinstrument.TheevidencealsoindicatesthattheinstrumentwaslastverifiedtobeoperableonApril13,1989,whenthesensinglineswereproventobeunobstructed.c)AnengineeringanalysisandaPRAwereperformedtoidentifythepotentialconsequencesofthisinstrumentchannelbeinginoperable.Theconclusionofthisanalysisestablishedthatneitheracontainmentbreachnoradditionalcoredamagewouldresultfromthissensinglinebeingcapped.d)ApressurizedairtestwasperformedonallotherUnit1andUnit2containmentpressuretransmittersensinglineswhichverifiedallsensinglineswereunobstructed.ThiswasperformedonMay7,1992.3~CorrectiveStestoAvoidFurtherViolationsa)ContainmentpressuresensinglinesonUnit1and2willbelabeledtopreventinadvertentcappingutilizingtheestablishedplantlabelingprocedures.Unit2willbecompletedpriortoMode4duringthe1992refuelingoutage.Unit1willbecompletedduringthe1993Springrefuelingoutage.b)PlantdrawingsonUnit2willberevisedasnecessarytoidentifyrequiredsensinglinecapconfigurationbyOctober14,1992.Unit1drawingswillberevisedbyAugust1,199 ~~Re:St.LucieUnit2DocketNos.50-389ReplytoNoticeofViolationInsectionReort92-07c)Anewcontainmentatmospherepenetrationinspectionprocedure(I&CProcedureNo.1400205)hasbeenwrittentoverifyselectedcontainmentinstrumentationpipingpenetrationswhichsensethecontainmentopenvolumeatmospherearefreeofobstructions.ThisinstrumentationincludesprocessinputsintotheContainmentHydrogenAnalyzer,ContainmentAtmosphereRadiationMonitoring,EngineeredSafetyFeaturesActuationSystem,ReactorProtectionSystem,andContainmentDifferentialPressureInstrumentation.Thisprocedurewillbeperformedpriortoareactorstartupfollowingeachrefuelingoutage.d)Mud-daubercaps(insectend-covers)havebeeninstalledonUnit2containmentpressuresensinglinestoprecludepersonnelfromcappingthem.Unit1installationwilloccurduringthe1993Springrefuelingoutage.4.FullcompliancewasachievedonMay7,1992,uponcompletionofcorrectiveactioninparagraph2.a).fCorrectiveAction3aforUnit2willbecompletedpriortoheatuptomode4duringthe1992refuelingoutage.CorrectiveAction3aforUnit1willbecompletedduringthe1993Springrefuelingoutage.CorrectiveAction3bforUnit2willbecompletedbyOctober14,1992.CorrectiveAction3bfor.Unit1willbecompletedbyAugust1I1993'orrectiveAction3dforUnit1willbecompletedduringthe1993Springrefuelingoutage.
