ATTACHMENTTOPLA4211FILER41-2PAGE1OF5REPLYTOANOTICEOFVIOLATIONViolation(387/94-16-03)10CFRPart50,AppendixB,CriterionXVI,"CorrectiveActions,"requires,inpart,thatin-casesofsignificantconditionsadversetoquality,measuresshallassurethatthecauseoftheconditionisdeterminedandcorrectiveactiontakentoprecluderepetition.Contrarytotheabove,onMarch4andMarch7,1994,thelicenseediscoveredthatfiredoor44wasblockedopenandtherequiredformNDAP-QA-0441whichauthorizesandrecognizestheconditionwasnotcompletedcontributingasignificantconditionadversetoquality.Additionally,priortotheseoccurrences,thelicenseeidentifiedseveralinstanceswherefiredoorswereblockedopenwithoutformNDAP-QA-0441beingcompleted.OnJuly20,1994,theinspectordiscoveredfiredoor44blockedopenwithouttherequiredapproval.Correctiveactionsforthesepreviouseventsdidnotprecluderepetitioninthattheinspectoridentified,again,onAugust23,1994thatfiredoor44wasblockedopenwithoutapproval.~Resense1.ReasonfortheViolationTheidentificationofstationdoorsasalsobeingfiredoorsisinadequate,andcombinedwithotherlabelingonthedoor,makesthedeterminationasafiredoordifticult.Theonlywaytoidentifyafiredoorisbythereddoorlabelwhichislocatedintheupper.cornerofthedoororbyutilizingNDAP-QA-0441,"FireProtectionSystemStatusControl."FireDoor844alsohadalabelaffixedtoitwhichstates"thisdoortoremainopen"(forventilationpurposes).Theintentwasthatfiredoor044onlybeheldopenbyitsfusiblelink.However,nonotificationwasavailableatthedoorwhichspecifiesthatthedoormaynotbeheldopenbyanymethodotherthanafusiblelin ATI'ACHMENTTOPLA-4211FILER41-2PAGE2OFS2.CorrectiveStesWhichHaveBeenTakenandtheResultsAchieveda.,ThestandusedtoblockopenFireDoor844wasremovedandthefusiblelinkassemblywasverifiedtoholdthedooropen.b.ThelabelonFireDoor844,thatstated"thisdoortoremainopen"wasremovedtoassistineliminatinglabelingconfusion.3.CorrectiveStesWhichWillBeTakentoAvoidFurtherViolationsa.Stationfiredoorswillbelabeledinlargelettersateyelevelasfollows:"FIREDOOR-DONOTBLOCKOPENWITHOUTAUTHORIZATION"b.Sitepersonnelwillbetrainedonthestatuscontrolrequirementsforstationfiredoors.TrainingofsitepersonnelwillbeconductedthroughGETTraining/RetrainingandPre-outagebriefings.4.DateofFullComlianceBasedon(2)abovePPkLisinfullcompliance.Labelingofstationfiredoors,asidentifiedin(3.a)abovewillbecompletedbyDecember31,1994.Trainingofstationpersonnelasidentifiedin(3.b)abovewillbecompletedbyApril10,199 AT1'ACHMENTTOPLAA211FILER41-2PAGE3OF58.Violalion(387/94-16-01)TechnicalSpecifications3.3.7.9,3.7.6.2and3.7.7requiresthatcontinuousfirewatchesbeestablishedwithinonehourwhenfiredetectionandsuppressionsystemsareinoperable.Contrarytotheabove,onAugust2,1994,at9:31p.m.,continuousfirewatcheswerenotestablishedwithintherequiredonehourfollowingthedeterminationthattheSimplexFireProtectionSystem(firedetectionandsuppressionsystem)wasinoperable.Specifically,foratleast10hoursfollowingtheevent,continuousfirewatcheswerenotimplemented.~ResenseReferenceLER50-387/94-012-00formoredetailsconcerningthiseventI.ReasonfortheViolationOnAugust2,1994,at2131hoursalightningstrikecausedacompletefailureoftheSimplexFireProtectionSystem.Availablepersonnelwereassignedrovingfirewatchdutiesintheaffectedareas.Continuousfirewatchescouldnotbeestablishedduetomanpowerlimitationsandtheextensivenumberofareasrequiringcoverage.Additionally,miscommunicationbetweentheSiteFireProtectionEngineerandShiASupervision,andinadequatecommunicationbetweenShiASupervisionandStationManagementcontributedtothefailuretoestablishcontinuousfirewatcheswithinonehour.ShiASupervisionaskedifhourlyfirewatcheswereacceptablegiventhesituationandtheFireProtection.Engineerrespondedthatitwas.However,ShiASupervisioninterpretedthistomeanacceptableforthedurationoftheLCO,andwhat'heFireProtectionEngineeractuallymeantbyhisresponsewasthatthiswasacceptableintheinterimuntiladditionalpersonnelwerecalledintomanthecontinuousfirewatches.InadequatecommunicationthenoccurredduringatelephoneconversationwhenShiASupervisionnotifiedStationManagementoftheevent.AssumptionsweremadebyStationManagementastotheactionplanandrecoveryeffortsfortheremainderoftheevening.Additionalquestioningand/orconversationwouldhaveledtoamoreappropriateresponse,specificallyrelatedtocontinuingeffortstostaffcontinuousfirewatchassoonaspossibl ATTACHMENTTOPLA-4211FILER41-2PAGE4OF52.ManaementExectationsa.StationManagement'sexpectationsregardingfireprotectionwascommunicatedtostationpersonnelviaaletterfromtheVicePresident-NuclearOperations.This'letterwasalsopublishedinthedepartmentalnewsletterwhichisdistributedtotheNuclearDepartment.Thisletterstatedexpectationswithrespecttofireprotectionprogramcompliance,procedureimplementation,andsystem/componentoperability.Italsoidentifiedprogrammaticshortcomings,statedactionsthatmustbeachieved,andactionsbeingtakentoachievestatedobjectives:TheletterconcludedbystatingthatSusquehannacannottoleratelessthanexcellentperformanceregardingfireprotection.-b.Additionally,Operation'sManagementissuedamemorandumtoShiftSupervisionidentifyingthefailuretomeetthecontinuousfirewatchLCOrequirementofthetechnicalspecifications,andtheirexpectationsthatrequirementsofthetechnicalspecificationsaretobeachieved.3.CorrectiveStesWhichHaveBeenTakenandtheResultsAchieveda.At0730hoursonAugust3,1994,itwasdeterminedthatcontinuousfirewatcheswererequiredandeffortswerestartedtosupportcontinuousfirewatches.Between0800and1210hoursonAugust3,1994,identificationofallfirezonesrequiringcontinuousfirewatcheswascompletedandstaffingof'thesezonesbegan.At1145hoursonAugust3,1994,theSimplexSystemwasrestored,andat1250hoursthefirewatchesestablishedforthiseventwereremoved.b.OperatingInstruction(OI-AD-013)wasissuedtoprovideshiftsupervisionguidanceintheeventofSimplexfailuresandtoassisttheoperatorthroughassessmentandcompensatorymeasuresdeemedappropriate.c.RequirementsforcontinuousfirewatcheshavebeenreevaluatedandupdatedconsistentwithTechnicalSpecifications.Thisresultedinareductioninthenumberofcontinuousfirewatchesrequired.d.AmemorandumhasbeenissuedtoOperationsshiftsupervisionemphasizingexpectationsregardingTechnicalSpecificationcompliance.(item2.b)e.Amemorandumhasbeenissuedtositepersonnel(letter)anddepartmentalpersonnel(newsletter)clarifyingexpectationsregardingfireprotection.(item2.a)

ATTACHMENTTOPLA4211FILER41-2PAGE5OF54.CorrectiveStesWhichWillBeTakentoAvoidFurtherUiolationsa.Trainadditionalon-shiftpersonneltobeabletoperformfirewatchduties.b.ProvideOperationscontrolroompersonneltrainingonSimplexsystemoperation,messagesandfailuremodesc.Reviewthiseventwithappropriateworkgroupsstressingtheimportanceofclearcommunications.d.EvaluateTechnicalSpecificationrequirementsforpostingfirewatcheswithinonehourandprocessappropriatechanges.e.AnEventReviewTeam(ERT)wasestablishedandperformedarootcauseanalysis.TheERTidentifiedseveralothercontributingfactorsinadditiontotheprimarycausesforthefailuretoestablishtherequiredfirewatchesandSimplexfailure(manpowerlimitations,extensivefirewatchareasrequiringcoverageandcommunications).TheseitemswillbedispositionedasdeterminedbytheERT.5.DateofFullComlianceBasedon(3.a)abovePALisinfullcompliance.Trainingofadditionalfirewatchesidentifiedin(4.a)abovewillbecompletedby12/31/94.Othertraining/reviewingactionidentifiedin(4.bandc)abovewillbecompletedby3/31/95.Evaluation/submittalofTechnicalSpecificationrevisions,ifnecessary,willbecompletedbyJune30,1995.
