This refers to your letter dated March 5,1985, in response to our letter dated January 21, 1985.
Appendix A Notice of Violation documented in your letter.Thank With regard to your response to Appendix 8 Notice of Violation, we could not determine the adequacy of your stated corrective action as your response letter did not address the root cause of the violation.
In a telephone conversation initiated by MR. R. R. Kefmig of this office on April 9, 1985 Mr. J. Basilio of your staff advised us that the root cause of violation was, inadequate mechan-isms for communication of post orders to security personnel and not ensuring that the security force supervisor was aware of compensatory posts.
Mr. Basilio further stated that, in addition to the corrective actions documented in your response letter, post orders are now required in writing and security force supervision is required, each hour, to reconcile the computer generated Ifst identified in your response letter.
