St.LucieUnits1and2DocketNos.50-335and50-389ReplytoNoticeofViolationInsectionReort95-01VIOLATIONA:TechnicalSpecificationsurveillancerequirement4.5.1.1.brequired,inpart,thatsafetyinjectiontanksbedemonstratedoperablewithinsixhoursofeachsolutionvolumeincreaseofgreaterthanorequaltoonepercentoftankvolumebyverifyingtheboronconcentrationofthesafetyinjectiontanksolution.Contrarytotheabove,onDecember31,1994,avolumeadditioninexcessofonepercentwasmadetothe1A2safetyinjectiontankwithoutverifyingtheboronconcentrationoftheresultantsolutionwithinsixhours.Asatisfactorysampleresultwasobtainedapproximatelyninehourslater.RESPONSEA:REASONFORVIOLATIONTherootcauseofthiseventwasthattheChemistrytechniciansampledthe1A1SafetyInjectionTank(SIT)inerrorafteravolumeadditiontothe1A2tank.Additionally,proceduresusedfortrackingeventdrivensurveillancestocompletionwerefoundtobeinadequate.Inanalyzingthisspecificevent,anindependentreviewofsurveillancesmissedduringthepastfiveyearswasconducted.Theresultsofthisanalysisrevealedaprocessweaknessintrackingeventdrivensurveillances.2.CORRECTIVESTEPSTAKENANDTHERESULTSACHIEVEDA.Operation'spersonnelidentifiedtheerrorduringareviewofthecontrolroomchronologicallogandcontactedChemistrytosamplethecorrect(1A2)SIT.B.Chemistrytechnicianssampledthe1A2SafetyInjectionTankwhichyieldedsatisfactoryresults.3.CORRECTIVESTEPTOAVOIDFURTHERVIOLATIONSA.TheOperationsDepartmentrevisedOP1-0410021andOP2-0410021"SafetyInjectionTankNormalOperatingProcedures",toincludeadatasheetfortrackingandverificationofSITboronsamples.ThisdatasheetwillremainwiththeshiftReactorOperatorsfromthetimeaSITisfilleduntiltheresultsfromChemistryaresatisfactorilyobtaine St.LucieUnits1and2DocketNos.50-335and50-389ReplytoNoticeofViolationInsectionReort95-01B.TheChemistryDepartmenthasenhancedtheircomputerprogramforloggingsampleresultstoincludeatimedependentnotificationsystemfortrackingsamplerequests.C.Atechnicalsubcommitteereviewedandanalyzedthisspecificeventincludingthegenericimplications.Theirfindingsconcludedthattheplant'smethodsforconductingscheduledsurveillancesensuredproperexecution,howeverforsurveillancesthatwereeventdriven,insufficientcontrolsexistedtotrackcompletion.Thefollowingcorrectiveactionsresultedfromthesubcommittee'sreview:OperationsDepartmenthasrevisedAP1-0010125AandAP2-0010125A,"SurveillanceDataSheets"toincludeaseparatedatasheetthattrackseventdrivenTSsurveillancestocompletion.2.TheplantLicensingDepartmentandTechnicalDepartmentwillreviewbothunits'echnicalSpecificationstoensurealleventdrivensurveillanceshaveimplementingprocedures.ThisactionwillbecompletedbyJuly30,1995.FullcompliancewasachievedonDecember31,1994withthecompletionofitem2.B.above.VIOLATIONB:TechnicalSpecification(TS)6.8.1.arequiredthatwrittenproceduresbeestablished,implemented,andmaintainedcoveringtheactivitiesrecommendedinAppendixAofRegulatoryGuide1.33,Revision2,February1978.AppendixA,paragraph1.dincludesadministrativeproceduresforprocedureadherence.AppendixA,paragraph9includesproceduresforperformingmaintenanc St~LucieUnits1and2DocketNos.50-335and50-389ReplytoNoticeofViolationInsectionReort95-01ProcedureQI5-PR/PSL-1,Revision60,"Preparation,Revision,Review/ApprovalofProcedures,"Section5.13.2statedthatallproceduresshallbestrictlyadheredto.LetterofInstruction2-LOI-T-89,Revision0,"DiagnosticTestingofLetdownLevelControlSystem,"requiredthatthepositiveandnegativeleadstoE/P2110QbereconnectedandindependentlyverifiedpriortoplacingvalveLCV-2110Qinservicefordiagnostictesting.Contrarytotheabove,onFebruary1,1995procedure2-LOI-T-89wasnotproperlyimplementedinthatthepositiveandnegativeleadstoE/P2110QwerenotproperlyreconnectedorIndependentlyVerifiedduetotheleadsbeingreversed,resultinginvalveLCV-2110Qnotopeningwhenitwasplacedinservice.Thisfailureresultedinalossofletdownflowandrequiredthatchargingbesecureduntiltheredundantlevelcontrolvalvecouldbeplacedinservice.RESPONSEB:'EASONFORVIOLATIONTherootcauseofthiseventwaspersonnelerrorbyanInstrumentandControl(I&C)Specialistinfailingtoadequatelyapplyself-checkingprinciples.Inaddition,therewasapersonnelerroronthepartofanIICsupervisortoperformaproperIndependentVerification.2.CORRECTIVESTEPSTAKENANDTHERESULTSACHIEVEDTheelectricalleadsfortheletdownvalvewereproperlyrelandedandindependentlyverifiedonFebruary1,1995.Thevalvewasthenplacedbackinserviceandoperatedproperly.3.CORRECTIVESTEPSTOAVOIDFURTHERVIOLATIONSA.Theindividualsinvolvedinthiseventwerecounselled,anddisciplinewasadministered.inaccordancewithcompany'olic St.Lu~ieUnits1and2DocketNos.'0-335and50-389ReplytoNoticeofViolationInsectionReort95-01B.C.TheplantTechnicalDepartmentisdevelopinganewAdministrativeProcedurewhichmoreclearlydefinesandconsolidatestheIndependentVerificationprocessforalldepartments.ThisprocedurewillbeimplementedbyApril28,1995.ThiseventandtherequirementsforIndependentVerificationasdelineatedinthenewadministrativeprocedurewillbeincorporatedintotraining.ThiswillbeincorporatedbyJune30,1995.D.Self-CheckingprincipleswillbeincorporatedintotheOntheJobTraining/TaskPerformanceEvaluationportionoftheMaintenanceContinuedTrainingProgram.ThiswillbeincorporatedbyJune30,1995.4.FullcompliancewasachievedonFebruary1,1995withthecompletionofitem2above.
