ATTACHMENTRESPONSETONOTICEOFVIOLATION0FINDING010CFR50,AppendixB,CriterionV,Instructions,Procedures,andDrawings,asimplementedbyapprovedFPLTopicalQualityAssuranceReportTQAR1-76ARev.15,TQR5.0,Instructions,Procedures,andDrawings,andfurtherimplementedbyTQARAppendixCcommitmenttoANSIN18-7,1976,paragraph5.2.7,MaintenanceandModifications,requirethatmaintenanceormodificationswhichmayeffectfunctioningofsafety-relatedstructures,systems,orcomponentsshallbeperformedinamannertoensurequalityatleastequivalenttothatspecifiedinoriginaldesignbasesandrequirements,materialsspecifications,andinspectionrequirements.10CFR50,AppendixB,CriterionVIII,IdentificationandControlofMaterials,PartsandComponents,asimplementedbyapprovedFPLTopicalQualityAssuranceReport1-76A,Rev15,TQR8.0,IdentificationandControlofMaterials,Parts,andComponents,requiresthatmeasuresbedesignedtopreventtheuseofincorrectmaterials,partsorcomponents.ThisisfurtherimplementedonsitebyprocedureQI-PR/PSL-8.1,Rev11,IdentificationControlofMaterials,Parts,andComponents.Contrarytotheabove,inthetwosafety-relatedrepairexamplescitedbelow,thelicenseefailedtocontrolmaterialandensurequalityatleastequivalenttothatspecifiedinoriginaldesignbasesandrequirements,materialsspecifications,andinspectionrequirementsinthat:1~TheequipmentmanualfortheReactorProtectionSystemchannel"C"cabinetfailedtospecifyapprovedreplacementwirematerial.Asaresult,onAugust21,1990,non-authorizedwirematerialwasinstalledintheUnit2cabinetduringthereplacementofthereactorcoolantpumpmodeselectorswitch.Thelicenseedidnotestablishqualificationbyothermeans.2~OnAugust24,1990,anon-authorizedsealantwasusedona1AComponentCoolingWater(CCW)pumpshaftmechanicalsealleak.Theunapprovedsealantwasappliedtothejointafteran"0"ringatthesealretainingcollarwascompressed,causingaminorleakatthecollar-to-pumpcasingjoint."0"ringreplacementwouldhaverequiredpartialpumpdisassembly.Ineachinstance,subsequentengineeringevaluationwasrequestedafterrepaircompletionandNRCinquiry.Theseevaluationsconcludedthattherepairswereacceptable,butthiswasnotknownatthetimeoftherepair.TheCCWsystemwasinoperationduringtheevaluationbuttheRPSchannelremainedoutofserviceuntilauthorizedmaterialwasobtainedandinstalle ATTACHMENTPage2of3RESPONSEi1~FloridaPowerandLightCompany(FPL)concurswiththeviolation.2~3~Thereasonfortheviolationwaspersonnelerrorcoupledwithinadequateprocedure.Thefollowingactionsweretakentocorrectthespecificitemsofnoncompliance:a.Theimproperwirewasreplacedwithvendorrecommendedwire.4~b.ThesealantusedontheComponentCoolingWaterPumpwasevaluatedbyFPLEngineeringanddeterminedtobeacceptableforuse.Thefollowingcorrectiveactionshavebeenorwillbeimplemented:a~InterimMeasures:b.Untiltheprocedurechangescanbemadeandtrainingconductedthefollowinginterimmeasureswillbeimplemented:(1)TheMaintenanceSuperintendenthasissuedaletterdescribingtherequirementsformaterialcontrol.Allpartsormaterialusedinsafety-relatedcomponentrepairswillbeverifiedtoconformtorequirements(i.e.TechnicalManuals,drawings,procedures,procurementspecifications,orstoresmaterialidentificationsheets).IfmaterialcannotbeverifiedworkwillbestoppeduntilawrittenEngineeringdispositionisreceived.(2)TheseinstructionswillbereviewedwithallMaintenancePersonneltoensurecompliance.ProgramCorrections:(1)Theworkcontrolprocedurewillberevisedtoprovidethedetailtoproperlyimplementtheplantqualityinstruct'ons.(2)Ourprogramstocontrolchemicalsandconsumablematerialswillbereviewedandrevisedasnecessary.(3)TrainingwillbeheldforallMaintenancePersonnelontherevisedmaterialcontrolprocedure ATTACHMENTPage3of35.Correctiveactionstatus.a~b.Correctiveactions3.a.and3.b.werecompletedbeforetheendoftheinspectionperiod.Correctiveaction4.a.willbecompletebyNovember30,1990.c~d.Correctiveaction4.b.(1)willbecompletedbyDecember31,1990.CorrectiveAction4.b.(2)willbecompletebyJanuary31,1991.e.CorrectiveAction4.b.(3)willbecompletebyFebruary28,199 tlII
