Reactor Project Section IA Inspection Summary Inspection on September 1 - November 30, 1981 (Report Nos. 50-329/81-22:

50-330/81-22)

Areas Inspected: Examination of site conditions; in-place storage and main-tenance of Limitorque valve operators; underated tenninal strips on valve operators; failure of Power-Strut spring lock nuts; cycle 2 SALP; and changes in site management. This inspection involved a total of 67 inspector-hours wisite by one NRC inspector including 0 inspector-hours onsito during off-shifts.

Results: No items of noncompliance or deviations were identified.

Functional or Program Areas Inspected a.

Site Tours At periodic intervals during the report period, tours of selected site areas were performed. These tours were intended to assess the cleanliness of the site; storage conditions of equipment and piping being used in site constr:ction; the potential for fire or other hazards which might have a deleterious effect on personnel and equip-ment; and to witness construction activities in progress.

In-Place Storage and Maintenance of Limitorque Motor Operatore Durino the reporting period, the Resident Inspector was notified by a ?"

.gion IV Vendor Inspector that one of the factory represen-tatives for Limitorque had questioned the adequacy of storage and maintenance conditions for Limitorque Valve Operators at the Midland S ite. A review of the existing storage and maintenance requirements was made and it appears that 1Lnit switch compartment heaters need not be energized provided the storage areas are warmed and/or con-tacts are protective coated. switch compartments open is being evaluated and the adequacy of the storage requirements and frequency of inspections is being reviewed. Therefore, this item is considered unresolved until these reviews and evaluations can be completed.

50.55(e) Item - Underated Terminal Strips on Limitorque Valve Operators During the reporting period, the licensee has detennined that several styles of terminal strips which are used in L?mitorque Valve Operators are either underated for the voltage application or have not been environmentally qualified for the service intend-ed.

The designation of these terminal strips are Buchanan 0824, Cinch Jones Series 140, Marathon 100 Series, Beau 7600 and Kulka 622. The above list of unsatisfactory tenninal strips indicates there may be weaknesses in the controls envoked during manufacture and/or service of Limitorque operators. Because of the industry-vide implications, the Resident Inspector informed the licensee that the installation of underated and/or unqualified terminal strips would be brought to the attention of the NRC Vendor Inspec-tors of Region IV.

Failure of Power-Strut Spring Lock Nuts

During the reporting period, the Resident Inspector was made aware through NRC internal menoranda that the Gulf States Utilities Company River Bend Nuclear Station had issued a 10 CFR Part 50.55(e)

item pertaining to failure of spring lock nuts used in Power-Strut supplied equipment. Therefore, the licensee was informed that the above referenced 50.55(e) item had been issued by Gulf States Utilities Company. The licensee pursued the item, issued a Safety Concern and Reportability Evaluation report, and was informed that the lock nuts used at River Bend Nuclear Station were isolated to a specific production lot of strut nuts (Lot No. PS-9227-1/2RS)

which were not used at the Midland Site. Power-Strut sent the Midland Site information pertaining to the 10 CFR Part 21 notifi-cation of the inability to confirm by testing the slip load values used when the nuts are installed to manufacturers specifications.

Cycle __2 Systematic Assessment of Licensee Perfortsnce (SALP)

During the reporting period, the Cycle 2 Systematic Assessment of Licensee Performan.e (SALP) was developed and NRC staff meetings were held to discuss the assessment. The scheduled meeting with the licensee was for November 13, 1981. However, on November 3, 1981, the NRC infonned the licensee that the meeting was cancelled and a later achedule would be established.

Changes in Site Management on October 1,1981, T. Cooke, Site Project Superintendent trans-ferred to the Consumers Power Company corporate offices la Jackson, Michigan as a Projects Administrator.

Mr. B. Peck became the Midland Site Construction Superintendent and essentially assumed those duties previously performed by Mr. Cooke.

On November 20, 1981, Mr. D. Turnbull, Site Project QA Superinten-dent became the Assistant Manager for Administration and Special Projects, and Nbssrs. B. Marguglio, Director, Enviromnental and Quality Assurance and W. Bird, Manager MPQAD assumed the responsi-bilities of the Project QA Superintendent. Although Messrs. Mar-guglio and Bird are from the corporate office, they have opted to manage the QA functions from the Midland Site offices.

Unresolved Items Unresolved items are matters about which more information is required in order to ascertain whether they are accepta' ale itens or items of noncom-pliance or deviations. Unresolved items disclosed during this inspcction are discussed in Paragraph 2.b.

Exit Interviews The Resident Inspector attended the exit interviews conducted by Region III Reactor Inspectors Messrs. R. Landsman, C. Jones and R.. Love on September 11, September 25 and October 9, 1981 respectively.

The Resident Inspector met with licensee representatives (denoted under Persons Contacted) on September 25, September 30, November 2 and December 2, 1981 to vuamarize the scope and findings of the inspection effort through November 30, 1981. The licensee acknowledged the findings reported herein.
