(Closed) Unresolved Item (315/78-31-01; 316/78-29-01):

Engagement of an independent consulting firm to conduct a review to identify all safety related equipment.

The review is continuing.

To date, the licensee reports all identified safety related equipment does have proper documenta-tion on file.

The review was initiated independently of Circular 78-08, therefore does not necessarily relate to it.

However, results of the review will be included in AEP's response to IE Bulletin 79-01, "Environmental Qualification of Class IE Equipment."

(Closed) Unresolved Item (315/78-13-02; 316/78-29-02):

Apparent conflict in test report CWAPD-332.

Kapton insulated conductors are protected from direct impingement of chemical spray by installation in stainless steel flood-up tubes.

During a loss of coolant accident, a maximum radiation exposure of 60 Mrads to Kapton insulated conductors would be encountered rather than the 150 Mrads as tested.

Environmental qualification of all cables by item number.

A more detailed review of cable disclosed power cable item numbers 347 and 399.

Item 347 was qualified by Franklin Institute Report FC3341.

Item 399 was qualified by Franklin Institute Report F-C3694.

(Refer to Okonite letter to AEP dated April 2, 1976.)

, Sample 4B, the type installed at D.

C. Cook, failed the high potential test.

However, sample 4B had

mil thick Okotherm (silicone) insulation; actual installation used 63 mil thickness.

Further, the cable is installed in conduit and acts as a non current carrying neutral of the hydrogen skimmer.

The installation is suitable for intended use.

(Closed) Unresolved Item (315/78-31-04; 316/78"29-04):

Applicability of nylon components in solenoid valves.

The licensee verified that solenoid valves with nylon components, including the types specified in reference 12 of IE Circular 78-08, were not installed in the D.

(Closed) Unresolved Item (315/78-31-05; 316/78-29-05):

Class IE instruments.

Rosemont resistance temperature devices (RTDs) were qualified by Westinghouse in topical report WCAP 9157.

(Closed) Unresolved Item (315/78-31-06; 316/78-29-06):

Motor operated valves and recirculation fan motors.

The containment recirculation fan motors were qualified by Westinghouse in topical report WCAP 7829.

WCAP 7829 was used in Advisory Committee Reactor Safety (ACRS) testimony for

. Unit. 2 during December, 1977.

Certification acutally linking the motors installed to those tested is pending.

Motor operated valves (MOV) were qualifed by several tests including Limitorque test-report 600456.

The most significant qualification attri-bute for an active MOV inside containment appears to be type RH motor insulation and gray Melamine material for limit and torque switches.

The licensee has verified that all active MOV's except two each in the RHR systems, have

"RH" stamped on the motor name plate.

Those valves without an "RH" stamp are being investigated for insulation type.

The lubricant tested during Limitorque tests was exxon Nebula EP-l.

AEP uses Mobilux EP-2 which was qualified to 2.4 x 10 rads by Customer Service Laboratory on June 2, 1971.

NOTE:

Cerification for the recirculation fan motors, and verification of MOV motor insulation will be reviewed relative to IE Bulletin 79-01.

No items of noncompliance were identified.
