This refers to the NRC Region I inspections conducted from May(20,1982 to

September 15, 1982, asdocumentedinReferences(1),(2),and 3),and

transmitted to you by our letters dated July 13, 1982, September 1.1982, and September 27, 1982.
l These inspections from May 20,1982 through Septem6er 15,1982 identified
several failures to adhere to procedural requirements. Those failures
'
resulted in not meeting Technical Specification requirements on Containment leakage, Containment valve lineup, and steam line flow instrumentation, in t
inoperability of the backup nitrogen supply for the Automatic Depressurization System, and in failure to make a report of Engineered Safety Feature actuation
to the NRC on time.
Overall, the core cause of the apparent violations seem to be multiple failures to adhere to established procedural controls. Such failures raise concern, not only about the training and attitude of the individuals involved in each j
instance, but about the effectiveness of their direct reviews and audits, and about accountability for procedure adherence at your facility, i
Therefore, we have scheduled an enforcement conference on January. 18,1982 in the NRC Region I office as discussed in our telephone corvarsation of January 12,.
1933. At this conference you should be prepared to address the aforementioned concerns, as well as the circumstances surrounding the events and the corrective and preventive measures which you have taken * will take to avoid recurrence.
