B.
Description of Particular Event or Circu= stance:
During the night of Aurast 28, 1972, the cpen end of a pneumatic hose, at 90 to 100 psig, beca e loose and whipped arcund in the pr'-*~/ inlet chamber of OTSG A.
The tube ends and velds en approxi=ately 1,000 tubes were da= aged in a 2-foot circular area.

Hampton stated that the damage was li=ited to the velds, and essentially
