Supplemental Response to Staff's Request Regarding Reactor Coolant System Heatup and Cooldown Weaknesses URC Combined Inspection 50-245/95-44; 50-336/95-44; 50-423/95-44 In a letter dated March 6, 1996,"8 the NRC Staff transmitted NRC l Inspection Report Nos. 50-245/95-44; 50-336/95-44; and 50-423/95-44. The report discussed significant weaknesses that l were identified at Millstone Unit No. 2 which contributed to l several recent failures to maintain plant parameters within required safety specifications. Specifically, controls for maintaining reactor coolant system temperatures within required limits during plant heatup and cooldown were not adhered to.

In response to a request that NNECO provide detailed assessment of the causal factors underlying these failures and a complete description and schedule of corrective actions, NNECO submitted a

,

letter dated April 19, 1996.23 In the letter, NNECO committed l

(Commitment No. B15653-1) to provide a supplemental response by May 30, 1996, that would contain an executive summary of the cooldown Event Review Team (ERT) findings and recommendations.

operating guidance and a discussion on the resolution of operator l burdens.

l The commitment date established was based on NNECO's expectations that the cooldown ERT report would be complete well before May 30, 1996. It was our intention that once complete, the l report would undergo management review and that an outcome of that review would be the establishment of the appropriate corrective actions in response to ERT findings and recommendations, i The purpose of this letter is to inform you that we have not yet l completed the ERT report. Resources necessary to finalize the i report were redirected to respond to emerging issues including those associated with the Millstone Unit No. 2 "B" emergency diesel generator failure which occurred on April 17, 1996. We believe that providing conclusions in advance of the ERT report completion and review would be premature in that the information contained could be incomplete or inaccurate. We will provide the l

previously committed information once the ERT report is complete, has undergone management review, and appropriate corrective actions have been identified. We anticipate that this will be completed in approximately one month. We regret any inconvenience that our inability to meet the previously-established date has caused the Staff.
