INSPECTION REPORT NO. 50-245/95-11; 50-336/95-11 AND 50-423/95-11 (REPLY)
Dear Mr. Feigenbaum:
This letter refers to your June 27,1995, correspondence, Reply to a Notice of Violation, in response to our letter, dated May 10,1995, regarding Millstone, Unit 2. This correspondence dealt with your failure to promptly identify and correct a design deficiency affecting the undervoltage modules in the engineered safeguards actuation system. We have reviewed this matter in accordance with NRC Inspection Manual Procedure 92903.
Thank you for informing us of the corrective and preventive actions documented in Attachment 1 of your letter. Their effectiveness will be examined during a future l
inspection of your licensed program.
We have also noted that, since the time of this inspection, additional system modifications have been implemented to improve the system performance. One result of the modifications was an improvement in the system immunity to the effects of electrical noise. In addition to the actions already implemented, we understand that you have initiated actions to replace the existing system with more modern equipment to eliminate long term operational and maintenance issues associated with equipment obsolescence.
