The inspection was an examination of the activities conducted under your license as they relate to radiation safety and to compliance with the Nuclear Regulatory Commission (NRC) rules and regulations and the conditions of your license. The inspection consisted of selective examinations of procedures and representative records, interviews with personnel, and observations by the inspector. Job Site - Swanson Road at CR 700 N, Portage, IN 1 Scope and Observations:

Midland Standard Engineering & Testing, a geotechnical engineering firm, was authorized by the State of Illinois to possess and use portable moisture density gauges containing byproduct material at its facility in South Elgin, and at temporary job sites in State jurisdiction. The company performed work at temporary job sites in NRC jurisdiction per a reciprocity filing submitted in April 2025 for the remainder of the calendar year. The scope of this inspection was limited to an evaluation of licensed activities (compaction testing) at a residential construction project in Portage, Indian The inspector visited the job site in Portage and met with the gauge user who currently covered the licensee's projects in Northwest Indiana. The individual was knowledgeable of radiation protection principles and regulatory requirements. Through demonstrations, the individual also demonstrated proficiency in portable gauge operation and implementation of the licensee's operating and emergency procedures. The gauge (a Troxler 3430 s/n 28381) and its transport case were in good condition and properly labeled; independent surveys in the vicinity of it were consistent with the radiation profiles in the applicable SSDR safety evaluation (NC-646-D-130-S). Adequate measures were available and used for securing and for blocking and bracing the transport case. The user kept complete and accurate shipping papers within arm's reach while driving, along with emergency response information, the licensee's operating and emergency procedures, and recent sealed source leak tests for the gaug The inspector also interviewed the licensee's RSO by phone to discuss additional aspects of the licensee's program, including gauge maintenance, accountability and emergency response measure No violations of NRC requirements were identified as a result of this inspectio NRC FORM 592M (10-04-2022)
