The inspection was an examination of the activities conducted under your license as they relate to radiation safety and to compliance with the Nuclear Regulatory Commission (NRC) rules and regulations and the conditions of your license. The inspection consisted of selective examinations of procedures and representative records, interviews with personnel, and observations by the inspector. Cardiovascular Consultants P.C. - 1730 45th Street, Munster, IN 1 Scope and Observations:
Medical Outsourcing Solutions, a medical services provider, was authorized by the State of Illinois to possess and administer diagnostic radiopharmaceuticals at numerous client locations across Illinois and at temporary job sites in State jurisdiction. The company performed work at temporary job sites in NRC jurisdiction per a reciprocity filing submitted in December 2024 for calendar year 2025. The scope of this inspection was limited to an evaluation of licensed activities (cardiac stress tests) at a client cardiology clinic in Munster, Indian The inspector visited the client's facility in Munster. Licensee staff adequately posted all areas of use, and maintained control and constant surveillance throughout the inspection. Independent surveys found no evidence of residual contamination, nor any ambient exposure readings above limits to the public in unrestricted areas. The inspector observed the preparation and administration of several diagnostic administrations of radiopharmaceuticals, which had been received by the licensee at its facility in Villa Park, Illinois, and transported by licensee staff to Munster. The inspector also observed functional checks of the licensee's survey instruments (a GM probe for exposure readings and an NaI probe for contamination). These were calibrated, operable, and appropriate for necessary survey tasks. Licensee staff were knowledgeable of radiation protection principles, wore personnel dosimetry as assigned, and utilized adequate ALARA practices to optimize occupational exposure The inspector also reviewed a selection of operational records, including diagnostic dose administration records, approved dose chart, instrument calibration and evaluation records, area survey results, package receipt results, shipping papers, and signed client agreemen No violations of NRC requirement were identified as a result of this inspectio Issuance of this report was delayed by the federal government shutdown from October 1 through November 12, 202 NRC FORM 592M (10-04-2022)
