(Violations and Corrective Actions)

Contrary to 10 CFR 39.65(a), on January 29, 2020, a logging supervisor employed by GeoLog Well Services, Inc. did not wear a personnel dosimeter while handling two sealed sources containing licensed materia As corrective action for this Severity Level IV violation, the logging supervisor wore a calibrated electronic dosimeter for the remainder of the work. The licensee also revised its pre-trip vehicle/ equipment checklist to include extra steps instructing crews to confirm the presence of dosimetry, a survey meter and long-handled tools before leaving the sho Statement of Corrective Actions l hereby state that, within 30 days, the actions described by me to the Inspector will be taken to correct the violations identified. [ZJ Temporary Job Site Inspection CR 750 Nat Cutoff Road, Washington Township, IN PROGRAM SCOPE This was an announced field inspection of a well services company authorized by the State of Illinois to store byproduct material at its facility in Wayne City, Illinois, and to use that material for well logging at temporary job sites in Illinois jurisdiction. The licensee conducted those activities at temporary job sites in NRC jurisdiction under the terms of a reciprocity request filed and approved in January 2020 for the calendar year. The scope of this inspection was limited to an evaluation of licensed activities (logging a wildcat natural gas well using Cs-137 and Am--241 sealed sources) in a rural area of southwestern Indiana, approximately a half mile west of the unincorporated community of Union, Indian PERFORMANCE OBSERVATIONS The inspector observed the conduct of well logging activities at the temporary job site near Union. The inspector observed the logging supervisor and his assistant assemble, use, and dismantle the tool string containing both sealed sources of licensed material, noting the use oflong-handled tools and other ALARA practices throughout. The inspector observed the logging supervisor perform surveys of the tools and job site using a calibrated survey meter, and conducted independent surveys to confirm the absence of residual contamination on or in the vicinity of the tools and the wellhea The inspector also conducted independent surveys of the sources in their locked, blocked and braced safes to verify compliance with DOT limits for radiation levels during transport on public highways. The inspector reviewed a selection of records available on the job site, including shipping papers, operating and emergency procedures, radiation surveys and instrument calibration records, current leak tests for both sources, and a copy of the company's State of Illinois radioactive materials license. The inspector did note that the job site coordinates on the lic~nsee's reciprocity change report were inaccurate; however this was because the client initially gave the licensee inaccurate informatio While observing the conduct of well logging activities, the inspector discovered that the logging supervisor was not wearing a personnel dosimeter while handling licensed material, contrary to IO CFR 39.65(a). The logging supervisor was initially unsure of where his badge had gone. The inspector detennined that the root cause of the violation was an oversight. As corrective action, the logging supervisor wore a calibrated electronic dosimeter for the remainder of the work, collecting approximately 0.04 millrem of dose during disassembly of the tool string and return of both sources to storage. The licensee also revised its pre-trip vehicle/ equipment checklist to include extra steps instructing crews to confirm the presence of dosimetry, a survey meter and long-handled tools before leaving the shop.
