COLUMBIA GENERATING STATION SIX-MONTH PERFORMANCE DATA REPORT July 1 to December 31, 2003 FITNESS FOR DUTY PROGRAM

SUMMARY

During the reporting period July 1 to December 31, 2003, the number of random tests performed met the number of required tests to achieve an annual rate equal to at least 50 percent of the unescorted access population. The rate of positive tests from random testing for this reporting period, July 1 to December 31, 2003, was 0.00 percent. The annual rate of positive tests from random testing for calendar year 2003 was 0.36 percent.

POSITIVE RESULTS Three (3) chemical tests yielded a positive result during this reporting period. Calculated from the positive tests recorded from January 1990 until December 2003, marijuana is the drug of preference, accounting for 61 percent of all the positive test results; cocaine is second, yielding 18 percent of the positive test results.

MANAGEMENT ACTIONS Pre-Access There were no pre-access positive test results during this reporting period.

Pre-Employment There were three (3) positive test results for contractor applicants for corporate resident visitor badges. The three applicants were applying for work on Energy Northwest property and were not applying for unescorted access to the Protected Area of Columbia Generating Station. Management was notified and the individuals' files were annotated of the positive test result and management's actions.

Random There were no random positive test results during this reporting period.

However there were three (3) random positive test results reported during the previous reporting period, January 1 to June 30, 2003, making the annual rate of random positive test results 0.36 percent.

Follow Up There were no follow-up positive test results during this reporting period. Presently, there are three licensee employees participating in the follow-up program.

For Cause/Observable Behavior There were no positive for cause test results due to observable behavior during this reporting period. There was one test administered for cause due to observable behavior with negative results.

The behavior observed was the accidental discharge of a handgun at the security training facility.

During handgun training with a class of temporary security officers, the instructor was demonstrating misfire clearing techniques with a handgun in the classroom. The instructor was using dummy ammunition in the handgun. During the demonstration, the handgun fired a live round, which struck and penetrated a door in the trailer where the class was being held.

For Cause/Post Accident There were no positive for cause test results due to post-accident testing during this reporting period. There were eight post-accident for cause tests administered post event/accident, all with negative results. No management actions were required. The events/accidents involved vehicle or equipment damage. The accidents included:

  • While backing up an Energy Northwest box truck to deliver furniture to the Plant Support Facility, the driver struck a parked vehicle.
  • A refuse truck driven by an Energy Northwest employee ran into a portable basketball hoop.
  • An Energy Northwest security employee parked a truck in the Protected Area Access Point (PAAP) parking lot. The employee opened the driver's door wide hitting the right passenger door of a parked car.
  • A forklift operator was transporting a compressor on a pallet through a rollup door in the warehouse complex. He noticed another vehicle parked outside the door which startled him, he activated the deadman brake which caused the compressor to tip forward and fall off the pallet.
  • An Energy Northwest truck was damaged when a water truck backed into it, causing damage to the right front fender, bumper, and light assembly. Both drivers of the vehicles were FFD tested.
  • An Energy Northwest employee was involved in a vehicle accident in the warehouse parking lot with another Energy Northwest vehicle. The accident -was classified as a "fender bender" with minimal damage to the vehicles.
  • An articulating lift turret came into contact with a parked truck. This incident occurred while performing routine maintenance (lamp replacements) on structures in the area.

There were no personal injuries involved in any of the events/accidents described above.

ADMINISTRATIVE ACTIONS There were no administrative actions taken during this reporting period.

EVENTS REPORTED TO THE NRC During this reporting period, there were no FFD events that were reportable in accordance with 10 CFR 26.73.

PROGRAM WEAKNESSES There were no programmatic weaknesses identified.

PROGRAM CHANGES The Behavioral Observation Program (BOP) was enhanced and training was revised to comply with requirements found in the "Compensatory Measures for Access Authorization" Order issued January 7, 2003. A new enhanced Behavioral Observation Program Site Wide Procedure was issued on November 12, 2003. A change management broadcast message was issued to all employees and contractor employees on September 24, 2003.

Beginning October 1, 2003, FFD/BOP training was removed from Protected Area Training (PAT) and became a separate course with its own exam. A separate FFD/BOP exam is now given with a minimum passing score of 80 percent. A change management broadcast message was issued to all employees and contractor employees on September 24, 2003.
