Failure to Re-baseline Valve Stroke Times as Required by ASME OM Code

Description

The inspectors identified a Green, non-cited violation of Palo Verde Technical Specification 5.5.8, Inservice Testing Program, which requires inservice testing of ASME Code Class 1, 2, and 3 components in accordance with the ASME Code for Operation and Maintenance of Nuclear Power Plants (OM Code). On October 22, 2017, the licensee failed to establish new stroke time reference values for Unit 1 safety injection (SI) valve 660 following maintenance which could affect the valves performance
