| Description | Licensee Technical Specification 5.7.1.b states in part that access to high radiation areas with dose rates not exceeding 1.0 Rem/hour at 30 centimeters from the radiation source shall be controlled by means of a radiation work permit that includes specification of radiation dose rates in the immediate work area and other appropriate radiation protection equipment and measures. Contrary to these regulations, on January 13, 2008, two quality control inspectors entered a pipe chase, a posted high radiation area, on the 1988 elevation of the auxiliary building using the wrong radiation work permit. The radiation work permit used by the licensee inspectors did not allow entry into a high radiation area. The violation was entered into the licensees corrective action program as CR 2008-00112. The finding was determined to be of very low safety significance because it did not involve: (1) ALARA planning and controls, (2) an overexposure, (3) a substantial potential for overexposure, or (4) an impaired ability to assess dose |
